保乳手术与改良根治术治疗早期乳腺癌患者的临床效果对比分析(1)
【摘要】 目的 研究保乳手術与改良根治术治疗早期乳腺癌患者的临床效果。方法 70例早期乳腺癌患者作为研究对象, 根据手术方法不同分为观察组(24例)与对照组(46例)。对照组采用改良根治术治疗, 观察组采用保乳手术治疗。比较两组各项手术指标, 并进行随访, 比较两组术后1年生存情况、术后1年复发情况、术后1年不良病症发生情况, 并由患者及家属填写问卷, 比较两组患者术后1年对美容效果的满意情况。结果 观察组患者手术时间为(140.5±20.4)min, 明显短于对照组的(174.5±27.6)min, 差异具有统计学意义(P<0.05);观察组患者术中出血量为(188.3±30.6)ml, 明显少于对照组的(286.5±43.8)ml, 差异具有统计学意义(P<0.05)。两组患者术后1年生存率、复发率、不良病症发生率比较差异无统计学意义(P>0.05)。观察组患者术后1年对美容效果的满意度为100.00%(24/24), 明显高于对照组的78.26%(36/46), 差异具有统计学意义(P<0.05)。结论 保乳手术应用到早期乳腺癌患者的治疗中能够有效缩短手术治疗时间, 减少手术治疗中的出血量, 美容效果得到患者及其家属的广泛认可, 有利于提高患者的生存质量, 与改良根治术相比综合应用价值更为显著, 值得推荐使用。

【Abstract】 Objective To study the clinical effect of breast-conserving surgery and modified radical mastectomy in the treatment of early breast cancer. Methods A total of 70 patients with early breast cancer as study subjects were divided by different surgical methods into observation group (24 cases) and control group (46 cases). The control group was treated with modified radical mastectomy, and the observation group was treated with breast-conserving surgery. Comparison were made on surgical indexes, 1 year-survival condition after operation, 1 year-recurrence condition after operation, occurrence of 1 year-adverse diseases after operation between the two groups. Results The observation group had obviously shorter operation time as (140.5±20.4) min than (174.5±27.6) min in the control group, and the difference was statistically significant (P<0.05). The observation group had obviously less amount of intraoperative hemorrhage as (188.3±30.6) ml than (286.5±43.8) ml in the control group, and the difference was statistically significant (P<0.05). There was no statistically significant difference in 1 year-survival rate, recurrence rate and incidence of adverse disease between the two groups (P>0.05). The observation group had obviously higher satisfaction with cosmetic effect 1 year after operation as 100.00%(24/24) than 78.26%(36/46) in the control group, and the difference was statistically significant (P<0.05). Conclusion Breast-conserving surgery can effectively shorten operation time of patients with early breast cancer, reduce amount of hemorrhage during operation. The cosmetic effect has been widely recognized by patients and their families, and it is conducive to improving the quality of life of patients.
