中药熏洗联合手法治疗手部屈肌腱修复术后粘连的疗效观察(1)
【摘要】 目的 研究分析針对手部屈肌腱修复术后粘连患者采取中药熏洗联合手法治疗的效果。方法 51例手部屈肌腱修复术后粘连患者, 按照单双号信封法分为观察组(26例)以及对照组(25例)。对照组患者均接受温水熏洗以及手法治疗, 观察组患者在对照组基础上加用中药熏洗治疗。比较两组患者治疗前后的手指总主动活动度(TAM)和患指功能恢复情况。结果 治疗前, 两组患者的手指总主动活动度对比, 差异无统计学意义(P>0.05); 治疗后, 两组患者的手指总主动活动度均大于本组治疗前, 且观察组(232.35±26.93)°大于对照组的(167.14±21.21)°, 差异均具有统计学意义 (P<0.05)。治疗4个疗程后, 观察组的患指功能恢复优良率为65.38%, 高于对照组的36.00%, 差异具有统计学意义 (P<0.05)。出院6个月后, 观察组的患指功能恢复优良率为88.46%, 高于对照组的64.00%, 差异具有统计学意义 (P<0.05)。结论 临床上治疗手部屈肌腱修复术后粘连患者时, 开展中药熏洗联合手法治疗效果良好, 患者的手指总主动活动度以及患指功能均显著改善, 具有较高的临床价值, 值得推广。

【Abstract】 Objective To study and analyze the effect of traditional Chinese medicine fumigation and washing combined with manual therapy in the treatment of adhesion after hand flexor tendon repair. Methods A total of 51 patients with adhesion after hand flexor tendon repair were divided into observation group (26 cases) and control group (25 cases) by envelope method. Patients in the control group received warm water fumigation and manual therapy, and patients in the observation group were treated with traditional Chinese medicine fumigation and washing on the basis of the control group. The total active movement (TAM) and functional recovery of affected fingers before and after treatment were compared between the two groups. Results Before treatment, there was no statistically significant difference in total active movement between the two groups (P>0.05). After treatment, the total active movement of the two groups was greater than that before treatment, and the observation group (232.35±26.93)° was greater than that of the control group (167.14±21.21)°, and the difference was statistically significant (P<0.05). After 4 courses of treatment, the excellent-good rate of functional recovery of affected fingers of the observation group was 65.38%, which was higher than that of the control group 36.00%, and the difference was statistically significant (P<0.05). 6 months after discharge, the excellent-good rate of functional recovery of affected fingers of the observation group was 88.46%, which was higher than that of the control group 64.00%, and the difference was statistically significant (P<0.05). Conclusion In clinical treatment of patients with adhesions after hand flexor tendon repair, the combination of traditional Chinese medicine fumigation and washing has a good effect, which can significantly improve the patient’s total active movement and the recovery of the affected finger.
