血液透析肾性贫血患者蔗糖铁治疗后骨髓象及铁代谢的临床观察(1)

     【摘要】 目的 观察应用蔗糖铁治疗维持性血液透析肾性贫血患者的骨髓象在治疗前后的变化,探讨蔗糖铁对肾性贫血患者骨髓和铁代谢的影响。方法 对22例维持性血液透析肾性贫血患者每周2次,每次100 mg蔗糖铁于血液透析时由动脉端输入,总量1000 mg。全部病例都合并使用红细胞生成素(EPO)治疗,剂量为120~150 U/(kg•周),皮下或静脉应用,观察治疗前后贫血纠正情况,骨髓红系各指标,铁代谢指标的变化,探讨蔗糖铁治疗的效果。结果 22例患者治疗5周后血色素(Hb)明显上升(P<0.01),红系骨髓象增生活跃程度比治疗前明显增强(P<0.01),且增幅高于红系血象各指标,血清铁、铁蛋白、转铁蛋白饱和度均升高(P<0.01)。结论 静脉注射蔗糖铁能保证EPO的铁需求,增强疗效。

    renal anaemia with Iron Sucrose Injection.And approach the influence of Iron Sucrose Injection to renal anaemia patients’ bone marrow and iron metabolism.Methods 22 cases of renal anaemia patients were treated with Iron Sucrose Injection.Intravenously guttae Iron Sucrose Injection from the artery side during hemodialysis(100 mg each time,twice every week,and the volume dose was 1000 mg).All the cases were injected erythropoietin(EPO)with the dose of 120 150U.kg 1.week 1.Then observe the change of anaemia,erythrocyte series of bone marrow and iron metabolism after the treatment,so as to approach the effect of the Iron Sucrose Injection.Results After 5 weeks’ treatment of the 22 patients,the haematochrome(Hb)improved obviously(P<0.01).The accrementition and activity of the erythrocyte series of bone marrow improved obviously than pretherapy(P<0.01).And the serum iron,ferritin and the degree of saturation of transferrin were all raised obviously(P<0.01).Conclusion Intravenously guttae Iron Sucrose Injection can ensure the iron requirement of the EPO and reinforce the therapeutic effect.
