【摘要】 目的 探讨阑尾系膜及动脉的解剖特点及处理技巧,方便腹腔镜阑尾切除术(LA)中阑尾系膜的分离,使得手术更简捷。方法 回顾分析2008年1月至2010年1月我院腹腔镜阑尾切除术,LA中于阑尾动脉近根部电凝止血,阑尾系膜电凝切开,阑尾残端结扎不包埋,电凝烧灼。结果 180例中,178例顺利切除阑尾,中转开腹2例。平均手术时间28.5 min。术后粘连性肠梗阻1例。无远期并发症。结论 LA安全可靠,操作更简捷。

    【Abstract】 Objective To discuss the anatomy specialty of appendix artery and velum,disposal skills,to make the separationofappendix velum easier and simplify the course of LA.Methods Review all of LA in our hospital from Jan 2008 to Jan 2010.In the course of LA,to coagulate appendix artery by electricity near the root,to carve appendix velum by electricity,don’t embed the extremity ofappendix,instead,burn the mucosa of the extremity.Results Among 180 patients,178 got LA,other 2 transit to OA(Openappendectomy),average.OPS time 28.5 min.1 patient got ileus,no long compliments.Conclusion The way introduced by the author is on the safe side and the operation is more prompt. ......
