【摘要】 目的 探讨三维对比增强MRA在下肢动脉病变血管成像中的临床应用价值。方法 对95例临床疑是下肢动脉疾病的患者行三维对比增强MRA检查,其中20例行DSA检查,以DSA为标准,评估CE-MRA检出下肢动脉狭窄的准确性。结果 双下肢主干动脉有1762节显示满意,满意显示率为97.62 %(1762/1805)。在所显示的动脉血管节段中,CE-MRA与DSA分别检出轻度狭窄30/28节、严重狭窄24/22节、闭塞12/11节。二者在轻度狭窄、重度狭窄、闭塞的一致率分别为93.33%、91.67%、91.67%。结论 下肢动脉三维对比增强MRA具有定位准确、无医源性损伤、安全迅速及可重复性等优点,是外科治疗下肢动脉疾病的重要依据,具有广阔的临床应用前景。

【Abstract】 Objective To explore the clinical value of three dimensional contrast enhanced MRA (3D CE-MRA) in lower extremities arteries disease. Methods 95 patients suspected with lower extremity arterial diseases underwent 3D contrast enhanced MRA. In which, 20 underwent DSA examination, look DSA as standard, assess the accuracy of CE-MRA on the stenosis of lower extremities arteries. Results 1762 arterial segments (about 97.62%,1762/1805)of the lower extremities main arteries were achieved by 3D CE MRA, was satisfactory. In these segments, mild stenosis were found 30 and 28 segments, severe stenosis 24 and 22 segments, and occlusion 12 and 11 segments by CE-MRA and DSA respectively. Compared with DSA, the rate of accordance in mild stenosis, severe stenosis and occlusion was 93.33%,91.67%,respectively. Conclusion 3D CE-MRA have an important role on the diagnosis of lower extremities arterial diseases, accuracy, noninvasion,safety, rapidness and reproducibility is it’s advantages. It is especially useful in arterial surgerical planning, and has big clinical application significance.

下肢动脉粥样硬化疾病是指动脉粥样硬化导致动脉狭窄甚至闭塞,使下肢组织出现慢性或急性缺血症状的疾病,其主要治疗方法是血管移植手术和经皮血管内支架成形术。对比增强磁共振血管成像(Contrast Enhanced Magnetic Resonance Angiography,CE-MRA)是安全可靠、快速无创的周围血管成像方法,尤其是自动移床下肢动脉CE MRA成像技术,扫描时间短、图像质量高,为周围动脉闭塞性疾病的诊断提供了准确可靠的影像信息[1] ......
