两种黏结剂修复楔形缺损的临床观察

[摘要] 目的 探讨全酸蚀黏结剂与自酸蚀黏结剂修复楔形缺损的临床效果。方法 选取2009年5月～2011年3月我院收治的楔形缺损患者60例(患牙182颗)，采用自身对照方法，随机分为全酸蚀组(左侧90颗)和自酸蚀组(右侧92颗)。比较两组修复成功率和敏感性。结果 修复1个月、6个月和12个月时，两组修复成功率比较，差异均无统计学意义(P>0.05)；全酸蚀组修复1个月时敏感发生率明显高于自酸蚀组(P<0.01)，修复6个月和12个月时差异无统计学意义(P>0.05)。结论 自酸蚀黏结剂能提供可靠的黏结效果，修复成功率高，且术后敏感更低，操作更加简便，值得临床推广应用。

Clinical Observation of Two Adhesive in Restoring of Wedge-shaped Defects

[Abstract] Objective To investigate the clinical effeacy of total-etching and self-etching adhesive in restoring of wedge-shaped defects. Methods Sixty patients(182 teeth)with wedge-shaped defect were selected from May 2009 to March 2011 in our hospital，using self-control，randomly divided 60 patients into total-etching group(90 teeth on left)and self-etching group (92 teeth on right). Success rate of restoration and sensitivity of two groups were compared. Results After 1 month，6 months and 12 months restoring，the success rate of restoration the two groups compared，the difference was not statistically significant (P>0.05)；After 1 month restoring，the sensitivity incidence rate in total-etching group was were significantly higher than that of Self-etching group(P<0.01)，there were no significant differences in restoring after 6 months and 12 months(all P>0.05). Conclusion The self-etching adhesive can provide reliable results，has a higher success rate of repairation and less sensitivity after the operation，and more convenient，worthy of clinical application.

楔形缺损是一种非龋性牙体硬组织疾病，是牙颈部硬组织缓慢磨耗所致的缺损，在老年人群中发病率高达90%以上[1]，常引起牙齿感觉过敏，严重者发生牙髓炎和根尖周炎，甚至牙体折断[2]，严重影响患者的生活质量。光固化复合树脂充填具有美观、保存牙体组织多、机械性能好等优点，是目前修复楔形缺损的主流方法[3]。成功的复合树脂充填除了依靠树脂材料的质量外，还要有较好的黏结系统。目前临床多应用全酸蚀黏结剂，近几年出现一种新型的自酸蚀黏结剂，操作更加简便，日益受到广泛关注。本文探讨两种黏结剂修复楔形缺损的效果，旨在为临床选择提供实践依据。
