通心络胶囊联合柳氮磺胺吡啶治疗强直性脊柱炎的临床观察(1)

[摘要] 目的 观察通心络胶囊联合柳氮磺胺吡啶治疗强直性脊柱炎的疗效。方法 36例患者随机分为两组，每组18例，均在常规应用非甾体抗炎药的基础上，治疗组予以通心络加柳氮磺胺吡啶(SASP)口服；对照组服用SASP；均治疗3个月，比较两组疗效及血沉和C反应蛋白指标的差异和不良反应。结果 治疗组患者的上述各观察指标与对照组比较，差异均有统计学意义(P<0.05)。结论 通心络胶囊联合SASP治疗强直性脊柱炎对缓解临床症状及改善有关实验室指标效果明显且迅速，具有一定优势，临床使用较安全。

Clinical Observation on Tongxinluo Capsule Combined with Salicylazosulfapyridine in Treatment of Ankylosing Spondylitis

[Abstract] Objective To evaluate the Tongxinluo capsule combined with salicylazosulfapyridine (SASP) for the treatment of patients with ankylosing spondylitis (AS). Methods Thirty-six patients were randomly divided into treatment group (routine administration of anti-inflammatory agents combined with Tongxinluo capsule and SASP， n=18) and control group (routine administration of anti-inflammatory agents plus SASP， n=18). The treatment duration was three months. Therapeutic effect， blood sedimentation， C-reactive protein and side effects were evaluated in both groups. Results Significant differences were detected between the indicators of treatment group and control group (P<0.05). Conclusion Tongxinluo capsule combined with SASP for the treatment of patients with ankylosing spondylitis showed remarkable therapy effects for the improvement of clinical symptoms and laboratory indicators， demonstrated its safety for clinical practice.

强直性脊柱炎(ankylosing spondylitis，AS)是以中轴关节慢性炎症为主，可累及内脏及其他组织的原因不明的慢性进展性全身性疾病，目前无特异性治疗，临床经典用甲氨蝶呤和SASP治疗，但其药物不良反应较大。本院2008年1月～2010年12月采用通心络胶囊联合SASP治疗AS，目的在于评价治疗效果，为临床提供满意的治疗方案，现报道如下。
1 资料与方法
1.1 一般资料
选择标准：36例强直性脊柱炎患者均符合1984年修订的强直性脊柱炎的纽约分类标准[1]，排除孕妇及哺乳期妇女，排除有磺胺类药物过敏史和精神病患者及有严重的其他疾病者。所有患者均有不同程度的单侧或双侧骶髂关节的破坏，HLA-B27均为阳性，也都未用肾上腺糖皮质激素。将所有患者随机分成两组：治疗组18例，男15例，女3例，年龄18～51岁，平均(22.8±5.9)岁，病程5个月～24年，平均(5.3±4.5)年；对照组18例，男16例，女2例，年龄16～52岁，平均(23.1±5.3)岁，病程4个月～25年，平均(5.1±4.4)年。两组临床资料无显著性差异(P>0.05)，具有可比性。
1.2 治疗方法
两组患者非甾体抗炎药常规用药，同时用胃黏膜保护剂。治疗组予通心络胶囊(由水蛭、全蝎、土鳖虫、蜈蚣、蝉蜕、赤芍、冰片等中药组成，石家庄以岭药业股份有限公司生产，国药准字Z19980015)，每次3粒，3次/d，联合SASP 3粒 ......
