腹腔镜胆囊切除术(LC)治疗急性胆囊炎的手术时机及效果观察(1)

[摘要] 目的探讨腹腔镜胆囊切除术(LC)治疗急性胆囊炎的手术时机，观察其治疗效果，并与常规开腹手术的治疗效果进行比较。方法 将我院2007年8月～2010年8月收治的急性胆囊炎患者70例随机分为A、B两组，A组患者行腹腔镜胆囊切除术(LC)治疗；B组患者行常规开腹手术治疗；比较术后两组的手术效果及并发症。结果与B组比较，A组患者手术时间、出血量及排气时间、抗生素使用时间、住院时间均明显少于B组，差异有显著性(P<0.05)；术后A组患者无一例发生切口感染、胆汁漏、胆管损伤，而B组发生切口感染2例，出血4例，胆管损伤2例，A组并发症的发生率(5.71%)与B组并发症的发生率(20.00%)比较，A组少于B组，差异有显著性(P<0.05)。 结论腹腔镜胆囊切除术(LC)治疗急性胆囊炎效果好，优于常规开腹手术的效果，并发症少，住院时间短，值得临床推广应用。

[Abstract] Objective To observe the therapeutic effect of laparoscopic cholecystectomy(LC) for the acute cholecystitis，and were compared with the conventional open surgery.MethodsAll 70 patients with acute cholecystitiswere divided into A and B groups in our hospital from August 2007 to August 2010. A group patientswere treated with laparoscopic cholecystectomy(LC).B group patients were treated by the conventional open surgery，and to compare the effect and complications after surgery. ResultsThe operation time，blood volume and exhaust time，antibiotic use，hospitalization time of A group patientswere significantly less than the B group(P<0.05). A group patients without postoperative a case of wound infection，bile leakage，bile duct injury，while the B group had wound infection in 2 cases，bleeding in 4 patients，bile duct injury in 2.And the complications rate of A group was 5.71%，group B was 20.00%，there was significantly difference(P<0.05). Conclusion The effect of laparoscopic cholecystectomy(LC)treatment acute cholecystitisis superior to the conventional open surgery，and had less complications and shorter hospital stay，and it wasworthy of clinical application.
