成人负压扁桃体电切术临床观察65例(1)

[摘要] 目的 评价负压扁桃体电切术的实用价值。方法 132例慢性扁桃体炎成人患者随机分成两组：治疗组65例，采用负压扁桃体电切术切除扁桃体125枚；对照组67例，采用剥离法切除扁桃体128枚。观察手术时间、术中出血、术后疼痛。结果 治疗组手术时间(3.87±2.58)min，术中出血(3.74±2.93)mL，术后第1天、第3天、第5天VAS疼痛评分分别为 (7.03±0.58)、(3.69±0.84)、(3.45±0.86)；对照组手术时间(13.27±5.71)min，术中出血(19.36±6.62)mL，术后第1天、第3天、第5天的痛觉-视觉模拟评分分别为(7.82±0.52)、(4.98±0.96)、(4.03±0.69)，具有统计学意义(P<0.01)。结论 负压扁桃体电切术比扁桃体剥离法切除更具优势。

Adult Negative Pressure Inhaled Tonsil Electricity Cut Method ：A Clinical Observation of 65 Cases

[Abstract] Objective To study the practical value of negative pressure electric tonsils cut method. Methods All 132 cases adult patients with chronic tonsillitis elected and divided into treatment group(65 cases)received negative pressure electricity cut method tonsils(125 removed) ,and control group(67 cases)received stripping method with tonsils(128 removed). The difference of operation time, blood loss in operation and postoperative pain were observed. Results The operational time in treatment group was(3.87±2.58)min, hemorrhage in operation was(3.74±2.93)mL, and the visual analog scale(VAS) were(7.03±0.58), (3.69±0.84)and(3.45±0.86)1,3,5 days after operation. The operational time in control group was(13.27±5.71)min, hemorrhage in operation was(19.36±6.62)mL, and the visual analog scale(VAS)were(7.82±0.52),(4.98±0.96)and(4.03±0.69)1,3,5 days after operation, and there was a statistically significant difference(P<0.01). Conclusion The negative pressure tonsil electricity cut method shows more attractively advantages than tonsils removed stripping method.

成人慢性扁桃体炎较为常见，且以剥离法扁桃体切除术为主要的根治手段[1]。术野深、窄，手术耗时、术中出血较多，剥离损伤咽部黏膜、创面大，术后疼痛、继发出血、感染为其主要并发症[2，3]。笔者近年来运用自制负压吸引套扎管结合改制电刀头，对该术进行改良，效果满意，现报道如下。
1 资料与方法
1.1 一般资料
2008～2011年3月本院门诊慢性扁桃体炎手术切除患者132例，均为成年男性。随机分为两组：治疗组 65例，年龄18～49岁，平均年龄27.9岁，采用负压扁桃体套扎电切术切除扁桃体125枚；对照组 67例，年龄20～47岁，平均年龄28.6岁， 采用剥离法切除扁桃体128枚。两组患者在年龄、病情、病程及术前常规检查等方面比较 ......
