早产儿甲状腺功能检测及临床意义(1)

     [摘要] 目的 探讨早产儿血清甲状腺功能检测及临床意义。方法 将我院2008年1月~2010年3月收治的早产儿100例分成两组:A组50例,胎龄<34周;B组50例,胎龄≥34周,对照组为同期出生的正常足月新生儿50例。应用放射免疫法对3组新生儿生后第1、8天血清三碘甲状腺原氨酸(T3)、甲状腺素(T4)、促甲状腺激素(TSH)水平进行检测。结果 三组新生儿T3、T4、TSH水平于生后1~8d呈下降趋势,A、B组T3、T4第1、8天均低于对照组,A组低于B组;生后第1天,A、B组TSH低于对照组,A组高于B组;生后第8天,A组TSH均高于B组和对照组,B组和对照组无明显差异。结论 早产儿生后甲状腺功能有暂时性低下,胎龄越小,功能越低,生后应激反应持续时间越长。

    Clinical Significance of Thyroid Function Detection in Premature Infants

    [Abstract] Objective To explore the thyroid function in premature infants. Mothods Serum triiodothyronine(T3),thyroxine(T4)and thyrotropin(TSH)were detected by radioimmunoassay(RIA)in 100 premature infants on the 1st and 8th days after birth from Jan 2008 to Mar 2010.They were divided into two groups according to the gestational age:50 cases in the group A,less than 34 weeks of gestational age,and 50 cases in group B,34-37 weeks of gestational age. 50 normal neonate composed control group. Results The concentrations of serum T3,T4,TSH showed a declining tendency in all neonates from the 1st day to the 8th day after birth. The concentrations of serum T3,T4 on the 1st and 8th day were significantly lower in group A and group B than those in the controls,and those in group A were also significantly lower than those in group B;On the 1st day after birth,the serum TSH were significantly lower in group A and group B than those in the controls,and those in group A were significantly higher than those in group B;On the 8th day after birth,the serum TSH in group A were higher than those in group B and controls,whereas the serum TSH were not significantly different between B and controls. Conclusion The thyroid function in the premature infants is transiently low. The shorter the gestational age,the lower the thyroid function and the longer the stress reaction.
