股骨近端锁定加压钢板和动力髋螺钉治疗老年股骨粗隆间骨折疗效比较(1)

[摘要] 目的 探讨股骨近端锁定加压钢板(LCP)和动力髋螺钉(DHS)治疗老年股骨粗隆间骨折的临床疗效。 方法 64例老年股骨粗隆间骨折患者随机分为DHS组(采用动力髋螺钉固定)和LCP组(采用股骨近端锁定加压钢板固定)，每组各32例，观察比较两组的手术时间、术中出血量、骨折愈合时间以及术后疗效。 结果 LCP组手术时间为(55.2±11.3)min，明显短于DHS组的(84.1±14.5)min(P<0.05)；LCP组术中出血量为(102.2±21.5)mL，明显少于DHS组的(260.7±42.1)mL(P<0.01)；两组骨折愈合时间比较差异无统计学意义(P>0.05)。两组临床疗效比较有显著性差异(χ2＝3.98，P<0.05)；LCP组术后骨折愈合优良率为96.9%(31/32)，明显高于DHS组的78.1%(25/32)(P<0.05)。 结论 与DHS术式比较，LCP术式治疗老年股骨粗隆间骨折具有手术时间短、术中出血量少、术后骨折愈合优良率高的特点，值得临床应用。

Comparison of clinical outcomes between proximal femur locking compression plate fixation and dynamic hip screw in treating old patients with intertrochanteric fractures

[Abstract] Objective To investigate clinical effet of proximal femur locking compression plate fixation (LCP) and dynamic hip screw in treating old patients with intertrochanteric fractures. Methods All of 64 old patients with intertrochanteric fractures were randomly divided into the DHS group and LCP group, with 32 patients in each group. The surgical time, bleeding amount, and fracture healing time and outcomes were observed and compared. Results In the LCP group, the surgical time was (55.2±11.3) min, which was significantly lower than that of the DHS group (84.1±14.5) min (P<0.05); the bleeding amount in the LCP group was (102.2±21.5) mL, which was significantly less than that of the DHS group (260.7±42.1) mL (P<0.01). The fracture healing time had no significant difference between two group (P>0.05). The clinical outcomes had significant difference between two groups (χ2＝3.98，P<0.05), and the healing ratio was 96.9% (31/32) in the LCP group, which was remarkably higher than 78.1% (25/32) in the DHS group (P<0.05). Conclusion Compared with DHS, the LCP could treat old patients with intertrochanteric fractures has less surgical time, bleeding amount, and high fracture healing ratio, thus worth clinical application ......
