[摘要] 目的 探讨三种不同类型抗菌药物治疗老年社区获得性肺炎的成本与临床效果关系。方法 将90例老年社区获得性肺炎患者随机分为A组(30例)、B组(30例)及C组(30例)，分别给予左氧氟沙星注射液、阿奇霉素注射液、头孢哌酮-舒巴坦钠注射液每日静脉滴注，三组总疗程均为10 d，运用药物经济学原理进行成本-效果分析。结果 3种药物治疗老年社区获得性肺炎的有效率分别为83.3%、76.7%、86.7%(P > 0.05)，细菌清除率分别为88%、79.2%、92.3%(P > 0.05)，且药物不良反应少，三组间比较差异无统计学意义；成本-效果比分别为(有效率) 9.9、14.7、20.0，(细菌清除率)分别为9.3、14.2、18.7，A组成本-效果比最小。 结论 左氧氟沙星注射液治疗老年社区获得性肺炎效果显著，安全可靠，值得基层医院临床推广应用。

Cost-effectiveness analysis of three different types of antibiotics in treatment of elderly patients with community acquired pneumonia

[Abstract] Objective To investigate the relationship between cost and clinical effectiveness of three different types of antibiotic in treatment of elderly patients with community acquired pneumonia. Methods All of 90 elderly patients with community acquired pneumonia were randomly divided into group A (30 cases)， B group (30 cases) and Group C (30 cases)，which were respectively treated with levofloxacin， azithromycin and cefoperazone - sulbactam for 10d，then used the pharmacoeconomic principles for cost-effectiveness analysis. Results For group A，B and C，effectiveness respectively were 83.3%，76.7%，86.7% (P>0.05)，bacterial clearance rates were 88%，79.2%，92.3%(P > 0.05)，and the incidence of adverse drug reactions were low，the difference among the three groups was not statistically significant; the Cost-effectiveness ratio respectively were (effectiveness) 9.9, 14.7, 20.0, (bacterial clearance rates) were 9.3，14.2，18.7，the cost-effectiveness ratio of group A was the smallest. Conclusion Levofloxacin treatment elderly patients with community acquired pneumonia was effective， safe and reliable， worthy of the primary hospital clinical application.
