新生儿黄疸与胆红素脑病的临床分析(2)
本研究中，病理性黄疸及胆红素脑病患儿的血清及脑脊液中胆红素水平均明显升高，胆红素脑病组血清胆红素(587.2±115.3) μmol/L，脑脊液中胆红素(14.7±3.8) μmol/L，均高于病理性黄疸组(血清及脑脊液中胆红素分别为(320.8±89.4) μmol/L和(7.5±3.1) μmol/L)。 说明血清及脑脊液胆红素可以作为胆红素脑病较敏感的诊断指标和病情评估指标。但同一患儿血清与脑脊液胆红素含量的比值同病情危重程度没有相关性。故对于新生儿黄疸的病情评判，采取动态血清胆红素水平监测，探索本地区小时胆红素百分位值可能更具有敏感性、客观性和可操作性。国内关于这方面的学术研究较多，如何整体把握病理性黄疸病情变化，对胆红素脑病早认识，早干预，减少致残率的同时，不过度浪费医疗资源，急需国内统一的诊疗指南出台，以更好地规范临床诊治。
