[摘要] 目的 探讨脑梗死死亡相关因素及死亡原因。 方法 对64例脑梗死死亡患者进行回顾性分析。 结果 在脑梗死死亡的相关因素中，既往有高血压病史者最多，其次为脑梗死和心肌梗死病史；合并症方面肺部感染相关性死亡47例，其次为合并脑疝及应激性溃疡者，且多于1周内死亡；梗死部位中后循环梗死者居多，尤其脑干梗死，并多于1周内死亡。 结论 脑梗死患者死亡的相关因素及死亡原因复杂，必须采取综合治疗才能降低病死率，改善预后。

The causes of death in patients with ischemic stroke： An analysis of 64 cases

[Abstract] Objective To study the death—related factors and causes of death in patients of ischemic stroke. Methods Sixty—four patients died from ischemic stroke were analyzed retrospectively. Results Hypertension was the most popular history among related factors for death after ischemic stroke， followed by a history of ischemic stroke. And myocardial infarction was common. 47 ischemic stroke—related—died patients accompanied with lung infection. Bisides， stress ulcer and cerebral hernia followed. Most of them died within 1 week. About causesof death， posterior infarct， especiallly brain stem infarction， was popular. And most of them died within 1 week. Conclusion The death—related factors and causes of death were complex， and the combined treatment should be applied to reduce the mortality and improve the prognosis.

脑梗死具有高发病率、高病死率、高致残率及高复发率的特点，据统计2008年我国脑梗死患者有700万人，每年新发病例约200万，并以每年近9%的速率递升。第三次死因抽样调查结果显示，脑血管病已成为第一位的死亡原因[1]，为降低脑梗死病死率，提高好转率，现对我院2007～2011年收治的64例脑梗死死亡的病例进行分析，探讨死亡原因，提出更有效的预防和治疗方法，改善预后。
1 资料与方法
1.1 一般资料
本组资料共64例，其中男40例，女24例。年龄46～ 92岁，平均年龄(73.1±10.2)岁。所有入选病例均经头颅CT或MRI检查证实为脑梗死，均符合1995年全国第四届脑血管病学会指定的诊断标准[2]。既往有高血压病史40例(62.5%)；脑梗死病史32例(50.0%)；心肌梗死病史17例(26.6%)；糖尿病病史12例(18.8%)。入院后合并脑疝者22例(31.3%)，并发肺部感染者47例(72.4%)，合并应激性溃疡者23例(35.9%)。
1.2 影像检查
64例患者均经过头颅CT或MRI检查证实为脑梗死，其中脑干梗死34例(53.1%)，基底节区脑梗死13例(20.3%)，脑叶梗死12例(18.8%)，前循环梗死28例(43.8%)，后循环梗死36例(56.3%)。
