锁定钢板与动力髋螺钉治疗老年股骨转子间骨折患者的疗效比较(1)

[摘要] 目的 比较锁定钢板与动力髋螺钉治疗老年患者股骨转子间骨折的疗效，旨在为临床治疗老年患者股骨转子间骨折提供指导依据。 方法 将2009年1月～2011年1月我院住院的28例锁定钢板治疗的老年股骨转子间骨折患者设立为A组，另选择同期行动力髋螺钉治疗的老年股骨转子间骨折患者28例设立为B组，比较两组患者的手术时间、术中出血量、住院时间、髋关节功能及并发症等。 结果 ①A组患者手术时间长于B组、术中出血量多余B组，差异有统计学意义(P < 0.05或P < 0.01)。但A组术后住院时间与B组比较，差异无统计学意义(P > 0.05)。②两组患者均随访1年，A组患者术后髋关节功能Harris评分显示，其优良率明显高于B组，差异有统计学意义(P < 0.05)。③A组患者术后发生并发症2例(7.2%)，B组患者术后发生并发症8例(28.6%)，两组患者术后并发症发生率比较，差异有统计学意义(P < 0.05)。 结论 锁定钢板与动力髋螺钉治疗老年股骨转子间骨折患者各具优缺点，临床医生应根据患者情况选择合适的内固定方式，从而发挥其优势。

The comparative effect of lock plate and the dynamic hip screw in treatment of the elderly patients with intertrochanteric fractures of femur

[Abstract] Objective To compare the effect between the lock plate and the dynamic hip screw in treatment of elderly patients with intertrochanteric fractures of femur. It was designed to provide guidance to clinical treatment of elderly patients with intertrochanteric fractures. Methods From January 2009 to 2011 January， 28 cases of hospitalized elderly femoral locking plate for treatment of intertrochanteric fractures in patients with established as group A， another to select the mobility in the same period hip screw in treatment of elderly intertrochanteric fractures in patients with established as group B， two groups were compared operative time， blood loss， hospital stay， hip function and complications. Results (1)A group of patients with operative time was longer than group B， blood loss， excess group B respectively， there were significant differences(P < 0.05 or P < 0.01). Group A， postoperative hospital stay compared with group B， the difference was not(P > 0.05). (2)All patients were followed for 1 year， group A patients with postoperative hip function harris score， its fine was significantly higher than group B， there are significant differences (P < 0.05). (3)A group of patients postoperative complications occurred in 2 cases(7.2%)， group B with postoperative complications occurred in 8 cases(28.6%) patients were postoperative complication rate was significantly different (P < 0.05). Conclusion Locking steel plate with the dynamic hip screw for the treatment of elderly intertrochanteric fracture patients each with advantages and disadvantages， clinicians should select the appropriate internal fixation according to the patient's condition， and thus play to their strengths.
