药品外包装和外标签存在的问题与建议(1)
[摘要] 随着医药工业的迅速发展，新药不断涌现，药品包装也越来越小，不同规格、剂型的药品越来越多，国家为完善和规范药品包装、标签及说明书也陆续出台了不少的规章制度，但在医院药品管理和使用中，经常发现因药品包装相似和包装方式不当而引发的问题仍然相当突出，经统计，药房调配处方发生差错的基本原因以药品外包装相似为主，居首位，而同一药品不同规格引起的差错， 占居第二。希望药品生产厂家在药品包装和外标签的设计上更加科学、规范，药品包装除了必须保证药品质量和方便运输外，还应多考虑方便临床使用和特殊群体患者的需求，以确保药品在使用中真正做到安全、有效、方便，减少因药品包装不当而引发的药疗事故。

[Abstract] With the rapid development of the pharmaceutical industry， new drugs are emerging， pharmaceutical packaging is getting smaller and smaller， with different specifications， the dosage forms of drugs are more and more countries to improve and standardize the drug packaging， labeling and instructions have been put forward by a lot of regulations system， but in the hospital drug management and use， often find the problems caused by drug packaging and improper packaging is still quite prominent， the main deployment of the basic reasons of prescription errors in statistical pharmacy drugs packaging similar in the first place. Different specifications of the same drugs caused the error， occupies the second. I hope the drug manufacturers in the design of drug packaging and the outer label is more scientific， standardized， pharmaceutical packaging must ensure that the quality of medicines and convenient transportation， but also give more consideration to facilitate the clinical use and the needs of special groups of patients， to ensure that the drugs in use truly safe， effective， convenient， reducing the medication caused the accident due to improper packaging of medicines.

药品外包装与外标签的科学化、规范化是药品质量和安全用药的重要因素。作为医院药剂科工作人员，在药品管理和使用中，发现因药品包装相似和包装方式不当而引发的问题较为突出，给药房工作顺利开展、方便患者使用、杜绝药疗差错带来隐患。现就有关问题作一探讨，希望能引起有关部门和生产厂家的重视，抓好药品包装改进，使之更加规范，更趋合理。
1 药品外标签基本信息存在的问题
药品的标签是指药品包装上印有或者贴有的内容，分为内标签和外标签，药品内标签指直接接触药品的包装的标签，外标签指内标签以外的其他包装的标签，药品外标签应当注明药品通用名称、成份、性状、规格、贮藏、生产日期、产品批号、有效期、批准文号、生产企业等内容[1]。上述规定的这些内容都是药师管理药品和调配处方必须要看的基本信息，然而现在部分厂家的药品外标签上的这些信息标注不规范，易引发药疗事故和医患纠纷。
1.1 包装规格信息难找、含义不清、容易误解
药品包装规格是药师发药的一个重要参数，药师调配处方时，首先要看药名，再看规格，根据医生处方按药品规格给患者配药。但目前，药品外标签上显示药品规格的位置不统一，药师需要在药盒的六个面上才能找到药品规格和包装数量，使工作效率大大降低。如东阿某厂生产的麝香壮骨膏的包装数量在有效期后面显示包装数量“5片/袋”；再如山东某制药厂生产的头孢地嗪钠，在药盒正面名称下写有0.5 g，如不细看或不熟悉的很容易误解为一盒0.5 g，侧面还有2支/盒字样，很容易造成发药差错。, http://www.100md.com(刘晓燕 王厚兴)
