泌尿系感染病原菌分布情况及耐药性分析(1)

     [摘要] 目的 了解我院泌尿系感染病原菌分布情况和耐药性变迁趋势,为临床用药提供实验室参考依据。 方法 用VITEK2-Compact全自动细菌分析仪,药敏同时兼做K-B法按最新版本的NCCLS折点对我院2011~2013年确诊泌尿系感染患者送检的10049份中段尿液标本进行检测。 结果 在10049份中段尿液标本中共检出病原菌3187株,阳性分离率为31.7%,主要病原菌检出顺序是大肠埃希菌1107(34.7 %)、凝固酶阴性葡萄球菌 379(11.9%)、肠球菌属275(8.63%)肺炎克雷伯菌234(7.34%)。肠杆菌科细菌对哌拉西林他唑巴坦和碳氢酶烯类抗生素亚胺培南、美洛培南耐药性较低,革兰阳性球菌没有发现对糖肽类抗生素万古霉素、替考拉宁的耐药株。 结论 泌尿系感染病原菌以革兰阴性杆菌大肠艾希菌为主,细菌对常用抗菌药物的耐药性较高。实验室应定时监测本地区泌尿系感染的主要病原菌菌群和其耐药性变迁,为临床合理选择抗生素提供参考。

    [Abstract] Objective To understand the hospital urinary tract infection pathogenic bacteria distribution and drug resistance change trend and provide laboratory reference for clinical medication. Methods Bacteria in VITEK2-compact automatic analyzer, susceptibility and K-B method according to the latest version of NCCLS fold point in our hospital from 2011 to 2013 patients with diagnosis of urinary tract infection were midway through 10049 urine specimens for testing. Results 3187 strains pathogenic bacteria were detected of 10049 urine specimens, separation of positive rate was 31.7%, the main pathogen detection sequence was e.coli 1107(34.7%), coagulase negative staphylococcus 379(11.9%) genera and 275(8.63%), klebsiella pneumoniae(7.34%) of 234. Enterobacteriaceae bacteria of piperacillin his temple and hydrocarbon enzyme alkenes antibiotics imine southern culture, beauty culture south resistance is low, did not find to sugar peptide antibiotics were gram positive coccus vancomycin resistant strains and their stead ning. Conclusion Urinary tract infection pathogens is given priority to with gram-negative bacterium escherichia athey bacteria, bacterial resistance to commonly used antimicrobial agents is higher. Laboratory shall regularly monitor the region the main pathogenic bacteria of urinary tract infection and their resistance to change, and to provide reference for reasonable selection of antibiotics.

    泌尿系感染(UTI)是指大量微生物在尿路中生长繁殖而引起的尿路炎症,可分为上尿路感染和下尿路感染,多见于成年女性,为最常见的感染性疾病[1]。尿液细菌培养是诊断UTI的金标准,药物敏感性试验对于UTI的治疗有指导作用,为了更加详细地了解UTI病原菌的分布与耐药情况,为临床合理选择抗生素提供实验室依据,减轻患者负担,提高治疗效果,延缓耐药株的产生,现将我院2011年1月1日~2013年12月31日确诊UTI患者的中段尿液培养检出病原菌的分布和药物敏感性试验结果进行分析,现报道如下。, http://www.100md.com(张智敏等)
