冠心病合并糖尿病冠状动脉造影特点及介入治疗的疗效观察(1)
[摘要] 目的 探讨冠心病合并糖尿病冠状动脉造影特点及介入治疗的临床疗效。 方法 将我科2011年8月~2015年3月收治的冠心病患者210例作为观察对象,根据是否合并糖尿病分为CHD+DM组50例与CHD组160例。比较两组冠状动脉造影特点与血管支架介入治疗效果。 结果 CHD+DM组单支病变发生率显著低于CHD组,三支病变发生率显著高于CHD组(P<0.01);两组患者双支病变、左主干病变、左前降支病变、左回旋支病变、右冠状动脉病变发生率比较,差异无统计学意义(P>0.05);CHD+DM组患者血管支架介入治疗病变个数、支架介入个数、术后残留程度明显高于CHD组(P<0.01);CHD+DM组患者球囊预处理最大扩张压力明显低于CHD组(P<0.05);两组患者总介入时间、球囊预处理最大扩张时间、术前狭窄程度比较,差异无统计学意义(P>0.05)。结论 冠心病合并糖尿病患者病变程度比非合并糖尿病组更为严重,但与病变位置无关;冠心病合并糖尿病患者的血管支架介入治疗近期效果不如非合并糖尿病组。

[Abstract] Objective To discuss the characteristics of coronary artery disease coronary and the clinical efficacy of interventional treatment. Methods All of 210 CHD patients in our department from August 2011 to March 2015 were treated as observation object, they were divided into CHD+DM group(n=50) and CHD group(n=160) according to whether diabetes. Coronary angiographic characteristics and interventional treatment effect were compared between two groups. Results The single-vessel disease incidence of CHD+DM group was significantly lower than CHD group, the three-vessel disease incidence of CHD+DM group were significantly higher than that of CHD group(P<0.01); The incidence of double vessel disease, left main disease, the left anterior descending coronary artery, the left circumflex artery lesions, the right coronary artery disease had no significant difference between two groups(P>0.05); The stent percutaneous coronary intervention lesions months number, stent number, degree of residual postoperative of CHD+DM group were significantly higher than the CHD group(P<0.01); The maximum expansion pressure of balloon pretreated in CHD+DM group was significantly lower than the CHD group(P<0.05); The total intervention time, the largest balloon dilatation pretreatment time, preoperative stenosis had no significant difference between two groups(P>0.05). Conclusion Coronary heart disease with lesions in diabetic patients is more serious than non-diabetic group, but regardless of the location of the lesion; The recent effect of interventional therapy for coronary heart disease complicated with diabetes mellitus is not as good as the combination of non diabetes mellitus., 百拇医药(孙立梅)
