感觉统合训练联合心理干预治疗儿童Tourette综合征患者的疗效(1)

     [摘要] 目的 探讨感觉统合训练联合心理干预治疗儿童Tourette综合征患者的疗效。 方法 选取儿科门诊治疗104例儿童Tourette综合征患者,随机分为观察组与对照组。两组患儿均予以氟哌啶醇治疗,对照组患儿予以单纯的心理干预,观察组患儿在对照组的基础上予以感觉统合训练,两组患儿均干预12周。观察两组患儿干预前和干预12周后Rutter儿童行为问卷评分的变化,并比较其临床疗效。 结果 干预12周后,两组患儿的Rutter评分较治疗前明显下降,低于干预前水平(P<0.05或P<0.01),且观察组患儿下降幅度较对照组更明显(P<0.05);同时观察组患儿治疗后的临床总有效率(94.23%)明显较对照组(80.77%)更佳。 结论 感觉统合训练联合心理干预治疗儿童Tourette综合征患儿的疗效确切,能明显改善其临床症状及行为问题。

    [Abstract] Objective To explore the curative effect of sensory integration training combined with psychological intervention in the treatment of pediatric Tourette syndrome. Methods 104 patients with pediatric Tourette syndrome who were admitted to the Pediatric Outpatient were selected and randomly divided into the observation group and the control group. Both groups were given haloperidol. The control group was given a simple psychological intervention, and the observation group was given sensory integration training on the basis of the control group. Two groups of children were intervened for 12 weeks. Changes of Rutter children's behavior questionnaire scores before the intervention and 12 weeks after the intervention were observed in both groups, and the clinical curative effect was compared. Results After 12 weeks of intervention, the Rutter scores in both groups were significantly lower than those before intervention(P<0.05 or P<0.01), and the decrease degree in the observation group was more obvious than that in the control group (P<0.05); the clinical total effective rate (94.23%) in the observation group after the treatment was significantly better than that in the control group(80.77%). Conclusion Sensory integration training combined with psychological intervention in the treatment of pediatric Tourette syndrome has an exact curative effect, which can significantly improve its clinical symptoms and behavioral problems.

    Tourette綜合征,又称多发抽动障碍,是一种伴有运动和行为异常的慢性神经精神障碍疾病,好发于儿童时期,病情顽固易复发,治疗与护理颇棘手[1,2]。Tourette综合征的发病机制较复杂,近年来研究发现社会心理因素在其发病中起极其重要的作用,以心理干预为核心的理念逐渐在儿童Tourette综合征治疗中的作用引起临床的重视[3]。感觉统合训练由Ayres博士创建,其主要作用是纠正感觉统合失调,患者通过训练对感觉刺激作适当反应而使感觉统合失调得到一定程度的纠正,国内关于儿童Tourette综合征患者的报道较少[4]。本研究观察感觉统合训练联合心理干预治疗儿童Tourette综合征患者的疗效,现报道如下。

    1 资料与方法

    1.1 一般资料

    选取2015年1月~2016年9月在我院儿科门诊治疗儿童Tourette综合征患者104例。纳入标准:①符合美国《精神疾病诊断统计手册(第4版)》中的诊断标准[5];②年龄5~14岁;③耶鲁综合抽动严重度量表(YGTSS)评分≥25分。排除标准:①偏头痛、手足徐动症、癫痫、肝豆状核变性、亨廷顿病、精神发育迟滞、精神分裂症和舞蹈病等;②有遗传代谢性疾病或药物过敏史。采用随机数字表将纳入的104例患儿分为观察组与对照组各52例。两组患儿在性别、年龄、病程与YGTSS评分等方面的情况比较相一致(P>0.05),具有可比性。见表1。本研究方案经医院伦理委员会批准,纳入患儿的父母均签署知情同意书。, 百拇医药(李娟 王央燕)
