杭州地区反复呼吸道感染患儿微量元素与免疫功能相关性分析(1)

     [摘要] 目的 探讨杭州地区呼吸道反复感染儿童微量元素与免疫功能的相关性。 方法 选择我院收治的反复呼吸道感染儿童116例为观察组,选择同期健康体检儿童110例为对照组。检测两组儿童血清微量元素(Ca、Zn、Fe、Mg、Cu)及免疫指标(IgA、IgG、IgM,CD4+T细胞、CD8+T细胞、CD4+/CD8+),并进行相关性分析。 结果 观察组患儿血清Ca、Zn、Fe水平显著低于对照组(P<0.05);观察组患儿IgA、IgG水平显著低于对照组,CD4+T细胞显著低于对照组,CD8+T细胞显著高于对照组(P<0.05),CD4+/CD8+显著低于对照组(P<0.05);呼吸道反复感染患儿Zn与IgA、IgG、CD4+T细胞呈显著正相关,Fe与IgA、CD4+T细胞呈显著正相关;微量元素缺乏与免疫功能低下是RRI发病的危险因素(P<0.05)。 结论 杭州地区反复呼吸道感染患儿微量元素及免疫功能下降,而Zn、Fe可能是导致患儿免疫功能低下的原因之一。

    [Abstract] Objective To investigate the correlation between microelements and immune function in children with recurrent respiratory tract infection in Hangzhou area. Methods A total of 116 children with recurrent respiratory tract infection admitted in our hospital were enrolled in this study as the observation group. 110 children in the same period were selected as the control group. The levels of serum trace elements (Ca, Zn, Fe, Mg, Cu) and immune indexes (IgA, IgG, IgM, CD4+T cells, CD8+T cells, CD4+/CD8+) in the two groups were measured and correlatively analyzed. Results The levels of serum Ca, Zn and Fe in the observation group were significantly lower than those in the control group (P<0.05). The levels of IgA and IgG in the observation group were significantly lower than those in the control group. The level of CD4+T cells in the observation group was significantly lower than that in the control group, and the level of CD8+T cells in the observation group was significantly higher than that in the control group(P<0.05). The CD4+/CD8+in the observation group was significantly lower than that in the control group(P<0.05). There was a significant positive correlation between Zn and IgA, IgG and CD4+T cells in children with recurrent respiratory tract infection. Fe and IgA, CD4+T cells were positively correlated. Trace element deficiency and immune function dysfunction were the risk of RRI risk factors(P<0.05). Conclusion The trace elements and immune function of children with recurrent respiratory tract infection in Hangzhou area are decreased, while Zn and Fe may be one of the reasons leading to the immune function dysfunction in children.

    反复呼吸道感染(recurrent respiratory infection,RRI)是临床常见肺科疾病,尤其在儿童高发。流行病学调查显示,浙江地区急性呼吸道感染发病率呈明显上升的趋势[1],这可能也是杭州地区儿童RRI高发原因之一。RRI发病机制复杂,其发病由多种因素影响,包括营养状况、病原体毒性与数量、外部环境及机体自身对入侵病原体的防御能力等[2-3]。儿童免疫系统发育不完全,易出现免疫功能低下,这被认为是RRI发生的最主要因素。有报道显示,RRI患儿存在微量元素异常表现,且RRI可能与此有关[4-6]。故本研究选择杭州地区116例RRI患儿为研究对象,检测其微量元素、免疫功能相关指标,分析其相關性,以期进一步明确RRI可能的发病机制,现报道如下。, 百拇医药(蔡姿秀 潘开宇)
