静脉腔内激光手术治疗下肢大隐静脉曲张的临床效果(1)
[摘要] 目的 观察采用静脉腔内激光手术治疗下肢大隐静脉曲张的临床效果。 方法 选取2016年1～12月我院收治的102例下肢大隐静脉曲张患者为研究对象，随机分为观察组与对照组，每组51例，观察组采取静脉腔内激光手术治疗，对照组采取高位结扎抽剥术治疗。对比两组患者住院时间、手术时间、切口长度、切口数量；对比两组患者治疗后临床疗效、不良症状发生率、治疗前后生活质量。 结果 观察组住院时间、手术时间、切口长度及切口数量、不良症状发生率均低于对照组，差异有统计学意义(P<0.05)；两组患者术后临床症状以及体征的改善情况无明显差异(P>0.05)；观察组临床治疗有效率、生活质量均高于对照组，差异有统计学意义(P<0.05)。结论 静脉腔内激光手术治疗效果好，可以有效节省住院、手术时间，术后并发症少，能有效提高患者生活质量，且患者满意度高，值得临床推广。

[Abstract] Objective To observe the clinical effect of intravenous laser surgery in the treatment of large saphenous varicose veins of lower limb. Methods 102 patients with large saphenous varicose veins of lower extremities were selected from Jan. to Dec. 2016 in our hospital and randomly divided into observation group(n=51) and control group(51). The observation group was treated with intravenous laser surgery， and the control group was given high ligation stripping surgery. The time of hospitalization， the time of operation， the length of incision and the number of incision were compared between the two groups. The clinical efficacy after treatment， the incidence of adverse reactions and the quality of life before and after treatment were compared between the two groups. Results The time of hospitalization， the time of operation， the length of incision and the number of incision and the incidence of adverse symptoms were lower in the observation group than those in the control group(P<0.05). There was no significant difference in the improvement of clinical symptoms and signs between the two groups(P>0.05). The effective rate of clinical treatment and quality of life in the observation group in the observation group were significantly higher than those in the control group， and the difference was significant(P<0.05). Conclusion Intravenous laser surgery has good treatment effect， which can effectively save the time of hospitalization and operation， with less postoperative complications. It also can improve the quality of life of patients， with high patients satisfaction， and is worthy of clinical promotion.

下肢大隱静脉属于最长的皮下浅静脉，从足背内侧到足背静脉，再到下肢内侧，上行到腹股沟[1]。下肢大隐静脉曲张在临床上较为常见，多发于小腿内侧，严重患者可能会发展到大腿内侧，能看到全下肢都有迂曲成团的静脉[2，3]。引发下肢大隐静脉曲张的原因有长期站立工作、重体力劳动、遗传因素等，发病机制是静脉壁薄弱、静脉瓣缺损及静脉内压增高导致[4]。下肢大隐静脉曲张的早期症状：站立太久会出现腿酸胀，有乏力、疲劳之感等，这些症状一般在清晨时段较轻，一天忙碌之后，晚上表现较为明显，患者足踝内侧有疼痛感，可以看到小腿有曲张静脉团块；下肢大隐静脉曲张的后期症状：站立太久双腿会有很重的肿胀感，影响行动，可以看到小腿布满曲张静脉团块，晚期会发展到足踝部溃疡，这些症状会严重影响了患者工作、生活、心理等[5-6]。目前对于这类疾病的治疗主要以手术为主，最常见的是下肢大隐静脉高位结扎抽剥术，但经过临床试验表明，此方法手术过程切口太多，且容易引发并发症[7-8]。随着医疗技术的发展，静脉腔内激光手术被广泛运用到治疗下肢大隐静脉曲张，为观察临床治疗效果，本研究以我院的下肢大隐静脉曲张患者为例，采用静脉腔内激光手术，效果显著，现报道如下。, 百拇医药(孙和平 王琛 徐小盛)
