胱抑素C及同型半胱氨酸在急性白血病诊疗中的应用价值(1)
[摘要] 目的 探讨血清胱抑素C(Cystatin C,Cys-C)和同型半胱氨酸(Hemocysteine,HCY)在急性白血病诊疗中的应用价值。 方法 选择血清尿素、肌酐浓度正常的33例急性粒细胞白血病患者(急粒组),34例急性淋巴细胞白血病患者(急淋组)及27例健康者(对照组),分析三组血清中的Cys-C、HCY含量差别。追踪急性白血病治疗后完全缓解患者和未完全缓解患者,分析两组血清Cys-C、HCY含量差异。 结果 急粒、急淋组与对照组经单因素方差分析显示HCY与Cys-C含量明显高于对照组,差异有统计学意义(P<0.001),进一步两两比较显示,急粒组与急淋组HCY差异无统计学意义,Cys-C差异有统计学意义(P=0.012)。追踪临床治疗,共收集急性白血病治疗后完全缓解患者28例,配对t检验显示治疗后同型半胱氨酸、胱抑素C明显下降;未完全缓解患者12例,配对t检验显示HCY、胱抑素C治疗前后差异无统计学意义。 结论 白血病患者檢测HCY、胱抑素C,有助于疾病诊断以及预后的评估。

[Abstract] Objective To investigate the application value of serum cystatin C(Cys-C) and homocysteine(HCY) in the diagnosis and treatment of acute leukemia. Methods 33 cases with acute myeloid leukemia with normal serum urea and creatinine concentration(acute myeloid group), 34 patients with acute lymphoblastic leukemia(acute lymphoblastic leukemia group) and 27 healthy subjects(control group) were selected. The differences in Cys-C and HCY levels in the serum of the three groups were analyzed. The patients with complete remission and patients with incomplete remission after acute leukemia treatment were tracked, and the differences in serum Cys-C and HCY levels between the two groups were analyzed. Results One-way analysis of variance between the acute lymphoblastic leukemia group, acute myeloid group and the control group showed that the levels of HCY and Cys-C were significantly higher than those in the control group, the differences were statistically significant(P<0.001), Further two-to-two comparison showed that there was no statistically significant difference in HCY between the acute myeloid group and acute lymphoblastic group, and the difference in Cys-C was statistically significant(P=0.012). After tracking clinical treatment, a total of 28 patients showing complete remission after acute leukemia treatment were collected. Paired t-test showed a significant decrease in homocysteine and cystatin C after treatment; there were 12 patients with incomplete remission. The paired t-test showed no statistically significant difference in HCY and cystatin C before and after treatment. Conclusion The detection of HCY and Cystatin C in the patients with leukemia is helpful for the diagnosis of disease and the evaluation of prognosis., http://www.100md.com(杨晓梅)
