[摘要] 通過介绍国内外慢性疾病管理模式，阐述基于互联网的慢病患者精细化管理模式的必要性。利用肾移植患者的高依从性，构建基于互联网的肾移植患者精细化管理模式，提高远程医疗随访、管理服务的自动化、智能化，使“互联网+医疗技术”能为更多居民与慢病患者提供全面而精准的服务。
