二氧化碳点阵激光联合果酸换肤治疗痤疮瘢痕的疗效和安全性分析(1)
[摘要] 目的 探討超脉冲二氧化碳(CO2)点阵激光联合果酸换肤治疗痤疮瘢痕的疗效和安全性。 方法 选择2017年11月～2018年3月本院收治的50例皮肤痤疮瘢痕患者作为研究对象，按随机数字表法将其分为治疗组(n=25)与对照组(n=25)，其中对照组患者接受单纯果酸换肤治疗，治疗组患者接受超脉冲CO2点阵激光联合果酸换肤治疗，治疗6个月后，采用痤疮瘢痕权重评分(ECCA)评估两组创面恢复情况，并根据拍片结果评定临床疗效，对比不良反应发生情况。 结果 治疗前，两组ECCA总分比较无显著差异(P>0.05)，治疗后，两组ECCA总分较治疗前比较显著降低，且治疗组ECCA总分显著低对照组(P<0.05);治疗后，治疗组治疗总有效率(96.00%)显著高于对照组(72.00%)，差异有统计学意义(P<0.05);治疗后，治疗组出现红斑、痤疮样疱疹、色素沉着等不良反应的总发生率(8.00%)略低于对照组(20.00%)，但差异无统计学意义(P>0.05)。 结论 针对痤疮瘢痕患者临床实施超脉冲CO2点阵激光联合果酸换肤治疗疗效显著，有利于加速其创面恢复，减少相应不良反应，因此推广应用价值较高。

[Abstract] Objective To observe the efficacy and safety of ultra-pulsed carbon dioxide(CO2) lattice laser combined with fruit acid peel treatment for acne scars. Methods 50 patients with skin acne scars admitted to our hospital from November 2017 to March 2018 were enrolled in the study. They were randomly divided into treatment group(n=25) and control group(n=25). Patients in the control group received simple fruit acid peeling treatment. The patients in the treatment group received ultra-pulsed CO2 lattice laser and fruit acid peel treatment. After 6 months of treatment， the acne scar weight score(ECCA) was used to evaluate the recovery of wounds in two groups. And according to the results of the film， the clinical efficacy was assessed. The occurrence of adverse reactions was compared. Results Before treatment， there was no significant difference in ECCA scores between the two groups(P>0.05). After treatment， the total ECCA scores of the two groups were significantly lower than those before treatment， and the ECCA total score of the treatment group was significantly lower than that of the control group(P<0.05). After treatment， the total effective rate of treatment in the treatment group(96.00%) was significantly higher than that in the control group(72.00%)， and the difference was statistically significant(P<0.05). After treatment， the total incidence of adverse reactions such as erythema， acne-like herpes， and pigmentation in the treatment group(8.00%) was slightly lower than that of the control group(20.00%)， but the difference was not statistically significant(P>0.05). Conclusion The clinical application of ultra-pulsed CO2 lattice laser combined with fruit acid peeling treatment for patients with acne scar has significant therapeutic effect， which is beneficial to accelerate the recovery of wounds and reduce the corresponding adverse reactions.
