低剂量MSCT测量COPD患者肺指标与肺功能检查的相关性(1)
[摘要] 目的 考察低劑量MSCT测量COPD患者肺测量指标与常规肺功能检查的相关性,评价低剂量MSCT的使用价值。 方法 回顾性选取2016年1月~2019年1月于本院呼吸内科同时行MSCT和肺功能检查的COPD住院患者以及健康受试志愿者各37例,利用64排128层螺旋CT行肺部MSCT扫描,自动化软件测量和计算容积指标;利用肺功能机测定患者肺功能情况,采用SPSS22.0软件对MSCT肺容积指标和常规肺功能指标进行比较并分析二者的相关性。 结果 两组肺容积指标中,相比对照组,观察组Vin、Vex、Vex/Vin明显更高,Vin-Vex更低,差异均有统计学意义(P<0.05);肺功能检查结果中,相比对照组,观察组TLC、RV、RV/TLC明显更高,VC、FEV1、DLCO、FEV1/FVC更低,差异均有统计学意义(P<0.05);相关性分析中,Vin与TLC相关性最好,Vex与RV相关性最好,Vex/Vin与RV/TLC、Vin-Vex与VC均呈正相关(P均<0.05)。 结论 低剂量MSCT测量COPD患者的肺指标测量值与肺功能检查部分指标具有较高的相关性,可明确COPD患者肺功能结构的变化,可作为辅助评估COPD患者肺通气功能的手段,且扫描曝光剂量低,值得在临床中推广使用。

[Abstract] Objective To investigate the correlation between lung index measured by low-dose MSCT and pulmonary function tests in patients with COPD, and to evaluate the application value of low-dose MSCT. Methods 37 patients with COPD inpatients and healthy volunteers who underwent MSCT and pulmonary function tests in our hospital from January 2016 to January 2019 were retrospectively selected. 64-row 128-slice spiral CT was used to perform pulmonary MSCT scanning. Automated software was used to measure and calculate the volume indicators. Lung function was measured by pulmonary function machine. SPSS22.0 software was used to compare MSCT lung volume index and conventional lung function index and to analyze the correlation between them. Results In the lung volume index of the two groups, the Vin, Vex, Vex/Vin in the observation group were significantly higher than those of the control group, and the Vin-Vex in the observation group was lower, with statistically significant difference(P<0.05). In the pulmonary function test results, the TLC, RV, and RV/TLC were significantly higher in the observation group than those of the control group, and the VC, FEV1, DLCO, and FEV1/FVC in the observation group were lower, with statistically significant difference(P<0.05). In the correlation analysis, the correlation of Vin and TLC was the best, Vex had the best correlation with RV, and Vex/Vin was positively correlated with RV/TLC, Vin-Vex and VC(P<0.05). Conclusion The lung index measured by low-dose MSCT in patients with COPD has a high correlation with some indicators of pulmonary function test, which can clarify the changes of lung function structure in patients with COPD. It can be used as a means of assisting in the assessment of pulmonary ventilation in patients with COPD.
