微创有限清理术治疗膝骨关节炎的临床效果及预后(1)
[摘要] 目的 探讨微创有限清理术治疗膝骨关节炎的临床效果及预后。 方法 將我院2015年6月～2018年12月的100例膝骨关节炎患者，随机分为对照组和观察组，每组50例。对照组予以关节镜下广泛清理手术，观察组采取微创有限清理术。比较两组疗效;膝关节疼痛消失时间、肿胀消失时间、住院时间;治疗前后视觉模拟疼痛程度评分、TNF-α水平、Lysholm 评分;并发症发生率。 结果 观察组的总有效率为98.00%，明显高于对照组的80.00%。治疗前两组视觉模拟疼痛程度评分、TNF-α水平、Lysholm评分比较，差异无统计学意义(P>0.05);治疗后观察组视觉模拟疼痛程度评分、TNF-α水平、Lysholm评分优于对照组(P<0.05)。观察组膝关节疼痛消失时间、肿胀消失时间、住院时间短于对照组(P<0.05)，两组均未出现严重的并发症。 结论 膝骨关节炎患者实施微创有限清理术可获得较好的效果，可有效缓解患者疼痛和肿胀，并控制炎症和改善膝关节功能，无严重并发症，具有良好的安全性。

[Abstract] Objective To investigate the clinical effect and prognosis of minimally invasive limited debridement for knee osteoarthritis. Methods 100 patients with knee osteoarthritis from June 2015 to December 2018 in our hospital were randomly divided into two groups， with 50 cases in each group. The control group underwent extensive debridement under arthroscopy for knee osteoarthritis patients， and the observation group underwent minimally invasive limited debridement for knee osteoarthritis patients. The curative effect， the disappearance time of knee pain and swelling， the length of hospital stay， visual analog pain score， TNF-α level， Lysholm score， and the incidence of complications before and after treatment between the two groups were compared. Results The efficacy of the observation group was 98.00%， which was higher than that of the control group(80.00%)， (P<0.05). There was no significant difference in visual analog pain score， TNF-α level， and Lysholm score between the two groups before treatment(P>0.05). The visual analog pain score， TNF-α level， and Lysholm score of the observation group were better than those of the control group after treatment(P<0.05). The knee joint pain disappearance time， swelling disappearance time， and hospital stay time were shorter in the observation group than those in the control group(P<0.05). And no serious complications occurred in both groups. Conclusion Minimally invasive limited debridement surgery for knee osteoarthritis patients can achieve better results， can effectively relieve patients' pain and swelling， and control inflammation and improve knee function without serious complications. It has good safety.
