1.5T MR腕关节平扫增强在痛风患者中的诊断效果及影像学特点研究(1)
[摘要] 目的 研究1.5T MR腕关节平扫增强在痛风患者中的诊断效果及影像学特点。 方法 选取2018年2月~2019年2月我院收治的痛风性腕关节炎患者10例,14个腕关节。对所有患者的14个腕关节都进行CT检查及1.5T MR检查。 结果 1.5T MR检查在骨内及软组织内痛风石、软组织肿胀及关节积液方面的病变检出情况显著多于CT检查(P<0.05)。14个腕关节全部表现出软组织肿胀及滑膜增厚,T1WI上表现出略微的低信号或等信号,T2WI上表现出略微的高信号或等信号;9个腕关节表现出骨质破坏侵蚀,T1WI上表现为弧形低信号,T2WI上表现为略微的高信号或者等信号(不均匀);9个腕关节表现出骨髓水肿,T1WI上表现出略低的信号,T2WI上和PDWI上表现为高信号;13个关节显示形成痛风石,单发性5个,多发性8个,共检查出痛风石27个,在T1WI上表现为略微的低信号或等信号,T2WI上和PDWI上表现出低混杂信号及低信号;12个腕关节表现出关节积液,T2WI上和PDWI上都表现出高信号。 结论 痛风性腕关节炎在1.5T MR当中表现多种多样,MR可以将炎症反应、滑膜厚度及软组织情况显示出来,对痛风性腕关节炎具有较高的诊断价值。

[Abstract] Objective To study the diagnostic effect of 1.5T MR wrist plain scan enhancement in gout patients and its imaging characteristics. Methods A total of 10 patients with gouty wrist arthritis admitted to Yiwu Central Hospital between February 2018 and February 2019 were selected in this study, including 14 wrist joints. CT and 1.5T MR were performed in all 14 carpal joints. Results 1.5T MR examination showed more lesions in the detection of intraosseous and soft tissue gouty stone, swelling of soft tissue and joint effusion as compared with CT scan(P<0.05). All of the 14 wrist joints showed soft tissue swelling and synovium thickening, and there was slight low signal or equal signal on T1WI and slight high signal or equal signal on T2WI. 9 of 14 wrist joints manifested as bone damage and erosion, and there was arc low signal on T1WI and slight high signal or equal signal(uneven) on T2WI. 9 of 14 wrist joints presented as bone marrow edema, and there was slightly low signal on T1WI and high signal on T2WI and PDWI. 13 of 14 wrist joints showed the formation of gouty stone, including 5 single, 8 multiple, and a total of 27 gouty stones were detected, and there was slight low signal or equal signal on T1WI, and low mixed signal and low signal on T2WI and PDWI. 12 of 14 wrist joints expressed as joint effusion, and there was high signal on T2WI and PDWI. Conclusion There are various manifestations of gouty carpal arthritis in 1.5T MR. MR can show the inflammatory reaction, synovial thickness and soft tissue, which has a high diagnostic value for gouty carpal arthritis.
