磁共振和双源CT在膝关节外伤中的诊断价值对比(1)
[摘要] 目的 对磁共振和双源CT在膝关节外伤中的诊断价值进行对比分析。 方法 整群选取该院2011年1月—2014年5月间收治的141例膝关节外伤患者的影像资料进行回顾性分析,以手术结果作为参照,对比两种检测方法的诊断准确率。 结果 双源CT与MRI对膝关节外伤的检出率分别为70.9%与94.3%,组间比较MRI的检出率显著高于CT,差异具有统计学意义(P<0.05)。以手术结果作为参照,对不同类型膝关节外伤的诊断准确率进行比较显示,MRI对于骨质损伤、韧带损伤、半月板损伤及关节囊积液的显示情况均优于双源CT,差异有统计学意义(P<0.05)。 结论 MRI对于膝关节外伤的各项检出率均高于双源CT。术前通过MRI全面检查掌握膝关节损伤情况,有助于帮忙骨科医生合理制定手术预案。

Comparison of Diagnosis Value of MRI and Dual-Source CT in Knee-Joint Trauma

[Abstract] Objective To analyze and compare the diagnosis value of MRI and dual-source CT in knee-joint trauma. Methods The images about 141 patients with knee-joint trauma treated from January 2011 to May 2014 in our hospital were retrospectively analyzed. According to the operation results, the detection accuracy based on two methods was compared. Results The detection rate of knee-joint trauma based on dual-source CT and MRI was 70.9% and 94.3%. The rate of MRI was higher than that of CT (P<0.05); according to comparison results of detection accuracy for different types of knee-joint trauma, the imaging of injury of bone, ligamentous injury, meniscus injury and drainage of articular capsule based on MRI was significantly better than that of dual-source CT (P<0.05). Conclusion The detection rate of knee-joint trauma based on MRI is higher than that of dual-source CT. Before surgery, the complete inspection of knee-joint injury based on MRI is good for the orthopedists to make the reasonable operation planning.

膝关节外伤骨科常见外伤之一,对其进行准确的影像学检查对治疗方案的制定具有重要意义[1]。普通X线片对于骨折部位、类型及移位情况无法准确全面地显示,CT对于软骨、半月板或韧带的显示度不佳[2]。MRI具有多方位、多参数成像的特点,能提供更好的病变部位信息,有效显示病变关节的解剖和病理变化,因此被广泛应用于膝关节外伤的影像诊断中。该研究整群选取该院2011年1月—2014年5月间收治的141例膝关节外伤患者的影像资料进行回顾性分析,以对MRI与双源CT在膝关节外伤中诊断价值进行对比。现报道如下。
1 资料与方法
1.1 临床资料
整群选取该院2011年1月—2014年5月收治的141例膝关节外伤患者为研究对象,所有患者均经关节镜或手术证实。其中男102例,女39例;年龄17~84岁,平均(43.5±6.3)岁;左膝关节96例,右膝关节45例。
1.2 方法
所有患者均同时接受MRI与CT扫描检查。①CT检查采用西门子SOMATOM Definition Flash炫速双源CT扫描仪。扫描范围以损伤膝关节为中心,上下各15 cm。扫描层厚为2 mm。②MRI检查采用GE Discovery MR750 3.0TMR机。患膝尽量伸直,外旋15°~20°。将其置于膝关节专用线圈内,常规采用SE、FSE、GRE和STIR序列行矢状位、冠状位及横轴位成像。
1.3 统计方法
采用SPSS20.0统计学软件进行数据分析处理。两种影像检查方法的各项检出率以百分比表示,组间比较采用行列表χ2检验,检验水准α=0.05。 (杨进军 何桂茹 班允清)
