ICU应用有创呼吸机患者下呼吸道感染的影响因素分析(1)
[摘要] 目的 分析ICU應用有创呼吸机患者下呼吸道感染的影响因素。 方法 随机选取2015年9月—2016年9月期间收治宜兴市人民医院的100例ICU应用有创呼吸机患者为实验对象,根据患者感染情况,分为感染组,即48例患者,未感染组,即52例患者,回顾性分析患者各项临床治疗,分析感染诱因、APACHE II评分。 结果 分析表格,可发现87.50%的下呼吸道感染患者待机时间不低于15 d,66.67%的患者基础病≤4种,38.33%的患者病情为V级。同时并发下呼吸道感染的患者APACHE II评分(28.41±3.46)分、抗酸剂使用率(89.58%)、胃内容物返流率(58.33%)高于未并发下呼吸道感染患者,白蛋白水平(31.85±2.69)g/L低于未并发下呼吸道感染患者。APACHE II评分、白蛋白水平(t=11.149 9、6.206 6;P<0.05)、抗酸剂使用率、胃内容物返流率(χ2=29.809 6、5.296 3;P<0.05)。结论 机械通气时间、基础病数量、病情等级、APACHE II评分、胃内容物返流率、抗酸剂使用率、白蛋白水平均是导致ICU应用有创呼吸机患者并发下呼吸道感染因素,通过分析好发因素后,实施相应的干预措施,方可降低感染率。

[Abstract] Objective To analyze the influence factors of lower respiratory infection of patients with invasive mechanical ventilation in ICU. Methods 100 cases of patients with invasive mechanical ventilation in ICU admitted and treated in our hospital from September 2015 to September 2016 were randomly selected and divided into two groups according to the infection situation, the non-infection group were 52 cases of patients, and infection group were 48 cases of patients, and various clinical treatment of patients was retrospectively analyzed and the infection causes and APACHE II score were analyzed. Results The analysis showed that the stand-by time of 87.50 percent of lower respiratory infection patients was not less than 15 days, and the basic disease of 66.67 percent of patients ≤4, and the disease of 38.33 percent of patients was V level, and the APACHE II score of lower respiratory infection patients was(28.41±3.46)points, and the use rate of antacid and gastric contents ebb-reflux rate were respectively 89.58% and 58.33%, which were higher than those of patients without lower respiratory infection, and the albumin level was(31.85±2.69)g/L, which was lower than that of patients with lower respiratory infection, and the APACHE II score and albumin level (t=11.149 9, 6.206 6;P<0.05), and the use rate of antacid and gastric contents ebb-reflux rate were respectively (χ2=29.809 6,5.296 3;P<0.05). Conclusion The mechanical ventilation time, basic disease number, disease classification, APACHE II score, gastric contents ebb-reflux rate, use rate of antacid and albumin level are the lower respiratory infection factors of patients with invasive mechanical ventilation in ICU, and we can implement the corresponding intervention measures to reduce the infection rate.
