集束化护理在降低难免压疮患者压疮发生率中的应用(1)
[摘要] 目的 对集束化护理措施在难免压疮患者护理中的应用方式以及效果进行详细探究。 方法 方便选取该院2016年8月—2017年8月收治的126例难免压疮患者参与该次研究，根据入院顺序分组，对照组69例、观察组57例。对于对照组患者，采用常规护理措施；对于观察组患者，采用集束化管理措施。对两组患者压疮发生率以及该次护理效果进行统计和比较。 结果 观察组患者中，Ⅰ期压疮患者3例，压疮发生率5.26%；对照组患者中，可疑的深部组织损伤3例，Ⅰ期压疮患者5例，Ⅱ期压疮患者4例，压疮发生率17.39%，组间差异有统计学意义(χ2=4.378，P<0.05)。结论 集束化护理管理在降低难免压疮患者实际压疮发生率方面效果显著，值得推广应用，以切实提升临床压疮预防水平，减少患者的痛苦。

Application of Cluster Nursing in Reducing the Incidence of Pressure Sore in Patients with Pressure Ulcers

[Abstract] Objective This paper tries to explore the application and effect of cluster nursing in the nursing of patients with pressure sore. Methods 126 cases of inevitable pressure sores conveniently admitted to the hospital from August 2016 to August 2017 participated in this study. According to the order of admission， 69 cases in the control group and 57 cases in the observation group. For the patients in the control group， the routine nursing measures were adopted， and the cluster management measures were adopted for the patients in the observation group. The incidence of pressure sore in the two groups and the effect of this nursing were analyzed and compared. Results The patients in the observation group， 3 cases of patients， the incidence of pressure ulcer was 5.26%； the control group of patients， 3 cases of suspected deep tissue injury， 5 cases of patients， 4 cases of bedsore of patients， the incidence of pressure ulcer was 17.39%； The difference was statistically significant (χ2=4.378， P<0.05)， with statistical significance. Conclusion Cluster nursing management is effective in reducing the incidence of pressure sores in patients with pressure sore. It is worthy of popularization and application to effectively improve the level of clinical pressure sore prevention and reduce the pain of patients.

現如今，难免压疮指的是在对患者进行健康状况评估、压疮风险因素评估后，对患者给予与其个人需求相一致的标准干预措施，然后对干预措施的实施效果以及影响进行监测和评价，同时，结合患者实际情况对干预方案进行调整，但是依然不可避免的会发生压疮。对于难免压疮患者，需要积极采取有效的预防压疮护理方案，尽量降低压疮发生率[1]。集束化管理是临床护理工作中的一种全新工作思路，其能够将循证基础治疗与护理措施进行有效结合，改善患者护理效果，集束化护理方式在难免压疮患者护理中的应用也逐渐受到广泛重视[2]。对此，在该次研究中，方便选取该院2016年8月—2017年8月收治的126例难免压疮患者作为研究对象，对集束化护理措施的应用方式以及应用效果进行详细探究，现报道如下。
1 资料与方法
1.1 一般资料
方便选取该院收治的126例难免压疮患者参与该次研究，其中脑梗塞患者38例，慢性阻塞性肺疾病患者27例，颅脑损伤昏迷并截瘫患者34例，骨折患者27例。根据入院顺序分组，对照组69例、观察组57例，所有患者及其家属均对该次研究知情，并签署同意书。对照组患者中，男32例，女37例，患者年龄在65～80岁之间，平均年龄(70.17±2.35)岁，观察组患者中，男28例，女29例，患者年龄在66～79岁之间，平均年龄(69.16±1.98)岁。两组患者在性别、年龄、疾病类型等一般资料方面比较，差异无统计学意义(P>0.05)。, 百拇医药(董棋香)
