全髋关节置换术和半髋关节置换术治疗老年股骨颈骨折的对比(1)
[摘要] 目的 对比分析全髋关节置换术与半髋关节置换术治疗老年股骨颈骨折的临床效果。方法 从2015年6月—2018年2月期间在该院接受治疗的股骨颈骨折老年患者中方便选取57例展开研究,根据治疗方式的不同分组,对照组28例行半髋关节治疗,观察组29例行全髋关节治疗,对比分析两组的手术情况、治疗效果以及术后并发症情况。结果 观察组住院时间和总输血量分别为:(22.21±3.12)d、(539.31±59.64)mL,均显著多于对照组的(19.97±3.05)d和(522.47±56.21)mL,差异有统计学意义(t=2.654 9、1.096 2,P<0.05);但是,观察组疼痛VAS评分、骨折愈合时间、髋关节功能评分均显著优于对照组,差异有统计学意义(t=-5.274 7、-6.652 8、4.667 4,P<0.05),且观察组的术后并发症发生率为3.45%,顯著低于对照组的17.86%,差异有统计学意义(χ2=3.076 3,P<0.05)。 结论 全髋关节置换技术成熟,除基础疾病多、心肺功能差的患者外,高龄患者都可选择行全髋关节置换手术治疗,其能早期重建关节功能,下地运动,减少长期卧床并发症。

Total Hip Replacement and Hemiarthroplasty for the Treatment of Femoral Neck Fracture in the Elderly

[Abstract] Objective To compare the clinical effects of total hip replacement and hemiarthroplasty in the treatment of femoral neck fractures in the elderly. Methods A total of 57 elderly patients with femoral neck fractures treated in the hospital from June 2015 to February 2018 were selected for study. According to different convenient treatment methods, 28 patients in the control group were treated with hemiarthroplasty. 29 patients underwent total hip arthroplasty. The operative status, treatment effect, and postoperative complications were compared between the two groups. Results The length of hospital stay and total blood transfusion in the observation group were (22.21±3.12) d and (539.31±59.64) mL, respectively, which were significantly more than those in the control group (19.97±3.05) d and (522.47±56.21) mL, the difference was statstically signficant(t=2.654 9、1.096 2,P<0.05). However, the VAS score, fracture healing time, and hip function scores in the observation group were significantly better than those in the control group. The difference was statstically signficant(t=-5.274 7, -6.652 8,4.667 4,P<0.05), and the postoperative complication rate in the observation group was 3.45%, which was significantly lower than the 17.86% in the control group,the difference was statstically signficant(χ2=3.0763,P<0.05). Conclusion The total hip arthroplasty technology is mature, in addition to patients with many basic diseases, poor cardiopulmonary function, elderly patients can choose to perform total hip replacement surgery, which can early reconstruction of joint function, lower sports, reduce long-term bed complications., http://www.100md.com(程应华)
