包头地区孕妇碘营养状况分析及其对新生儿足跟血促甲状腺激素水平的影响分析(1)

     [摘要] 目的 探究包头地区孕妇碘营养状况分析及其对新生儿足跟血促甲状腺激素水平的影响。方法 采用随机分层按比例抽样的方法,于2017年1月—2018年1月选取包头市城市和农村共260名孕妇进行尿碘测定,并追踪测定其分娩的新生儿足跟血促甲状腺激素值。 结果 3组孕妇尿碘在<150 μg/L、150~300 μg/L、>300 μg/L占比分别为53.86%、30.71%、15.43%,差异无统计学意义(P>0.05),不同尿碘指标(<150 μg/L、150~300 μg/L、>300 μg/L)的孕妇所生新生儿在血促甲状腺激素水平分别为(2.15±0.23)、(2.13±0.25)、(1.91±0.19)μIU/mL,差异有统计学意义(P<0.05),在新生儿血促甲状腺激素水平>5 mU/L的比例占比上,虽不同尿碘指标的孕妇所生新生儿之间差异无统计学意义(P>0.05),但<150 μg/L的孕妇所产的新生儿有部分高于3%。 结论 包头地区孕妇碘缺乏现象较为普遍,临床应针对性给予相关措施,扩大宣教力度,将尿碘检测纳入孕期常规检测,根据检验情况适量补充,以提升人口素质,减少新生儿智力发育异常等问题。

    [Abstract] Objective To investigate the iodine nutritional status of pregnant women in Baotou area and its effect on neonatal heel thyroid hormone levels. Methods A random stratified proportional sampling method was used to select 260 pregnant women in urban and rural areas of Baotou City for urinary iodine determination, and to track the neonatal heel thyroid stimulating hormone values. Results The urinary iodine levels of the three groups were <150 μg/L, 150-300 μg/L, and >300 μg/L, respectively, which were 53.86%, 30.71%, and 15.43%, respectively. There was no significant difference (P>0.05), and different urinary iodine indicators (<150 μg/L, 150~300 μg/L, and >300 μg/L) the blood thyroid stimulating hormone levels of neonates born to pregnant women were (2.15±0.23)μIU/mL, (2.13±0.25)μIU/mL, (1.91±0.19)μIU/mL, respectively, P<0.05, in the proportion of neonatal blood thyroid hormone levels >5 mU/L, although there was no significant difference between the newborns born to pregnant women with different urinary iodine indicators (P>0.05), but <150 μg/L the proportion of newborns born to pregnant women was higher than 3%. Conclusion The iodine deficiency in pregnant women in Baotou area is more common. The clinical measures should be targeted to expand the education. The urine iodine test should be included in the routine test during pregnancy, and supplemented according to the test to improve the quality of the population and reduce the mental development of newborns.

    碘是甲狀腺合成甲状腺激素所必需的微量元素,对人体的生长发育起到至关重要的作用,甲状腺激素分泌不足会影响大脑发育[1-2]。妊娠可导致妇女甲状腺功能发生生理性的显著变化,女性在妊娠期间,碘的生理需求量比正常人高1/3~1倍,母体的甲状腺发生一系列的生理适应性变化,伴有缺碘现象[3-4]。为进一步探讨包头地区孕妇碘营养状况,该研究采用随机分层按比例抽样的方法,于2017年1月—2018年9月选取包头市城市和农村共260名孕妇进行尿碘测定,旨在探讨包头地区孕妇及新生儿情况,现报道如下。
