右美托咪定联合曲马多预防瑞芬太尼麻醉后痛觉过敏的临床效果分析(1)
[摘要] 目的 观察与分析右美托咪定联合曲马多预防瑞芬太尼麻醉后痛觉过敏的临床效果观察。 方法 方便选取该院2017年11月—2018年11月收治的70例瑞芬太尼麻醉患者,单双数法随机分为实验组与对照组,各35例。对照组患者采用曲马多预防瑞芬太尼麻醉后痛觉过敏,实验组在对照组基础上加用右美托咪定。观察与比较两组患者的麻醉苏醒时间、VAS评分、拔管时间等。 结果 实验组的麻醉时间、麻醉苏醒时间、拔管時间分别为(2.58±0.41)min、(10.84±3.49)min、(7.51±2.40)min,均快于对照组,差异有统计学意义(t=10.734、10.941、12.526,P<0.05)。实验组的VAS评分好于对照组,差异有统计学意义(P<0.05)。实验组的不良反应发生率低于对照组,差异有统计学意义(P<0.05)。 结论 右美托咪定联合曲马多预防瑞芬太尼麻醉后痛觉过敏的临床效果显著,可以减轻患者的疼痛感,减少患者的不良反应。

[Abstract] Objective To observe and analyze the clinical effect of dexmedetomidine combined with tramadol in preventing hyperalgesia after remifentanil anesthesia. Methods 70 patients with remifentanil anesthesia admitted to this hospital from November 2017 to November 2018 were conveniently selected. The single and double method was randomly divided into experimental group and control group, with 35 cases in each group. In the control group, tramadol was used to prevent hyperalgesia after remifentanil anesthesia. The experimental group was given dexmedetomidine on the basis of the control group. The anesthesia recovery time, VAS score, and extubation time of the two groups were observed and compared. Results The anesthesia time, anesthesia recovery time and extubation time of the experimental group were (2.58±0.41)min, (10.84±3.49)min and (7.51±2.40)min, respectively, which were faster than the control group (t=10.734, 10.941, 12.526,P<0.05), and the difference was statistically significant. The VAS score of the experimental group was better than that of the control group(P<0.05), which was statistically significant. The incidence of adverse reactions in the experimental group was lower than that in the control group, and the difference was statistically significant(P<0.05). Conclusion Dexmedetomidine combined with tramadol prevents the clinical manifestations of hyperalgesia after remifentanil anesthesia, which can reduce the pain of patients and reduce the adverse reactions of patients.

瑞芬太尼是临床上常用的麻醉剂,麻醉效果好,可控性强,具有诸多优点。但有研究指出,如果持续给药不仅会导致患者出现麻醉后痛觉过敏等症状,而且会对使用者的血压、中枢神经系统、胃肠道造成较大的影响,影响治疗效果[1]。曲马多属于非阿片类中枢性镇痛药,依赖性小,镇痛效果佳,临床上常使用曲马多预防、减缓瑞芬太尼麻醉后痛觉过敏症状。有研究表明,右美托咪定对预防瑞芬太尼麻醉后痛觉过敏有良好的效果[2]。为了研究右美托咪定联合曲马多预防瑞芬太尼麻醉后痛觉过敏的临床效果,该文对该院2017年11月—2018年11月收治的70例瑞芬太尼麻醉患者进行对比分析,现报道如下。, http://www.100md.com(李慧静)
