睑板腺按摩在白内障术后干眼症治疗中的疗效分析(1)
[摘要] 目的 观察睑板腺按摩治疗白内障术后干眼的疗效。方法 方便选择2016年6月—2017年2月，行白内障超声乳化手术后明确干眼症患者262例(262眼)，其中，男性160例，女性102例：年龄56～85岁，平均年龄72岁。将其随机分成两组，分别为治疗组130例，对照组132例。治疗组行睑板腺按摩、睑缘清洁、睡前典必殊眼膏搽睑缘、人工泪液治疗，对照组仅用人工泪液治疗。观察记录治疗前后两组的泪膜破裂时间、角膜荧光染色及眼部症状评价。结果 治疗前两组患者的BUT[(3.54±1.26)s vs (3.65±1.10)s]，CFS[(1.87±0.69)分 vs (1.89±0.89)分]以及眼部症状评价水平差异无统计学意义(t=0.185、0.331、0.335，P=0.846、0.725、0.524>0.05)，治疗后治疗组的各项指标BUT(8.55±1.52)s，CFS(0.65±0.38)分改善水平均优于对照组BUT(4.56±1.21)s，CFS(1.55±0.45)分，差异有统计学意义(t=12.062、5.386，P<0.05)结论 睑板腺按摩在治疗白内障术后干眼症，效果显著，适合临床推广应用。

[Abstract] Objective To observe the effect of meibomian gland massage on dry eye after cataract surgery. Methods From June 2016 to February 2017， convenient selected 262 patients (262 eyes) with dry eye syndrome were identified after phacoemulsification. Among them， 160 males and 102 females： aged 56-85， with an average age of 72 years. They were randomly divided into two groups， 130 cases in the treatment group and 132 cases in the control group. The treatment group underwent sacral gland massage， sacral margin cleaning， pre-sleeping dynasty， and the artificial tear treatment. The control group was treated only with artificial tears. The tear film rupture time， corneal fluorescence staining and ocular symptom evaluation were recorded before and after treatment. Results There was no statistically significant difference in the level of BUT [(3.54±1.26)s vs (3.65±1.10) s]，CFS[(1.87±0.69)points vs (1.89±0.89)points]and evaluation of ocular symptoms before treatment， (t=0.185，0.331，0.335，P=0.846，0.725，0.524>0.05). After treatment， the BUT (8.55±1.52)s and CFS (0.65±0.38)points in the treatment group improved the treatment level better than the control group BUT (4.56±1.21)s， CFS (1.55± 0.45)points. The difference was statistically significant (t=12.062，5.386，P<0.05). Conclusion The meibomian gland massage in the treatment of dry eye after cataract surgery， the effect is significant， suitable for clinical promotion and application.

隨着白内障超声乳化手术普及，全国开展的手术量也逐年增长。白内障手术切口损失了部分角膜缘细胞及结膜杯状细胞，术前、术中及术后各种药物的使用均会影响泪膜的稳定性，导致患者术后视力改善了，却出现畏光、干涩、流泪等不适[1]。这种白内障手术源性干眼是比较常见的一种术后并发症。严重的甚至影响术后视力和视觉质量。导致白内障手术的治疗效果不理想，甚至会出现部分患者拒绝再次接受第二眼的白内障手术。该次在2016年6月—2017年2月在应用人工泪液的基础上联合睑板腺按摩治疗130例白内障术后干眼症取得良好效果，报道如下。, 百拇医药(苏颖丹 刘向 张艳春)
