体位引流排痰预防脑卒中恢复期患者医院获得性肺炎的研究(1)
[摘要] 目的 探討体位排痰手法对脑卒中恢复期卧床患者医院获得性肺炎(HAP)的预防效果。方法 方便选取该院2018年3—12月脑卒中恢复期患者231例,随机分为实验组(n=118)和对照组(n=113),实验组在常规护理的基础上给予体位引流排痰治疗,对照组在常规护理的基础上给予排痰机机械振动排痰治疗,比较两组患者医院获得性肺炎的发生率。 结果 实验组患者男女性别比、脑卒中类型比分别为77∶41、94∶24,对照组患者对应数值分别为73∶40、91∶22,两者均差异无统计学意义(χ2=0.011,0.027,P=0.917,0.869);实验组年龄为(59±17)岁,对照组年龄为(60±16)岁,两组年龄差异无统计学意义(t=0.031,P=0.975);实验组病程为(56±23)d,对照组病程为(61±21)d,两组病程差异无统计学意义(Z=-0.142,P=0.886);实验组HAP的发病率为8.4%,对照组为18.6%,两组HAP发病情况进行比较,结果差异有统计学意义(χ2=5.077,P=0.024)。 结论 体位排痰可减少脑卒中恢复期卧床患者医院获得性肺炎的发生率,减轻病人的经济负担,促进患者的早日康复。

Study on Prevention of Hospital-acquired Pneumonia in Patients with Stroke Recovery by Positional Drainage

[Abstract] Objective To investigate the preventive effect of postural drainage and sputum manipulation on hospital acquired pneumonia (HAP) in patients with stroke during convalescence. Methods Convenient select a total of 231 patients with recovery from March to December 2018 in our department were randomly divided into experimental group (n=118) and control group (n=113). The experimental group was given the position on the basis of routine nursing, and the drainage control was performed on the drainage; the control group was given the mechanical vibration and drainage treatment of the drainage machine on the basis of routine nursing. The incidence of hospital acquired pneumonia was compared between the two groups. Results The male-to-female gender ratio and stroke type of the experimental group were 77∶41 and 94∶24, respectively. The corresponding values of the control group were 73∶40 and 91∶22, respectively. There was no significant difference between the two groups (χ2=0.011, 0.027, P=0.917, 0.869); the age of the experimental group was (59±17) year, and the age of the control group was (60±21) year. There was no significant difference in age between the two groups (t=0.031, P=0.975); the course of the experimental group was (56±23) days, and the course of the control group was (61±21) days. There was no significant difference in the course of disease between the two groups(Z=-0.142, P=0.886).The incidence of HAP in the experimental group was 8.4%, and that in the control group was 18.6%. The incidence of HAP in the two groups was compared, and the difference was statistically significant (χ2=5.077, P=0.024).
