NIPPV模式和NCPAP模式对新生儿呼吸窘迫综合征的治疗对比分析(1)
[摘要] 目的 觀察并分析无创间歇正压通气(NIPPV)模式与经鼻持续呼吸道正压通气(N-CPAP)模式对新生儿呼吸窘迫综合征(NRDS)的影响。方法 方便选取该院自2018年1月—2019年12月收治的100例新生儿呼吸窘迫综合征(NRDS)患儿作为观察对象,随机分为试验组(50例)与对照组(50例)两组。两组患儿分别给予NIPPV模式与N-CPAP模式治疗。通过观察两组患儿治疗后的呼吸频率、撤机时间、总吸氧时间与血氧状态,对比两组患儿临床治疗效果。结果 试验组RR(55.35±5.32) L/min、撤机时间(6.84±2.07)d、总吸氧时间(7.22±3.39)d 优于对照组(t=3.524、6.781、7.056,P<0.05);试验组治疗后PaCO2(48.37±2.31)mmHg、PaO2(75.67±3.25)mmHg及P/F(289.75±16.89)指标优于对照组(t=7.161、8.934、11.821,P<0.05)。 结论 无创间歇正压通气(NIPPV)模式治疗新生儿呼吸窘迫综合征,明显优于经鼻持续呼吸道正压(N-CPAP)模式,有效改善了患儿的肺功能与血氧状态,缩减撤机时间及总吸氧时间,建议临床推广应用。

[Abstract] Objective To observe and analyze the effects of noninvasive intermittent positive pressure ventilation (NIPPV) mode and nasal continuous positive airway pressure (N-CPAP) mode on neonatal respiratory distress syndrome (NRDS). Methods A total of 100 infants with neonatal respiratory distress syndrome (NRDS) admitted to the hospital from January 2018 to December 2019 were conveniently selected as observation objects, and they were randomly divided into two groups: experimental group (50 cases) and control group (50 cases). Two groups of children were treated with NIPPV mode and N-CPAP mode. By observing the respiratory rate, weaning time, total oxygen inhalation time and blood oxygen status of the two groups of children after treatment, the clinical treatment effects of the two groups of children were compared. Results The experimental group RR (55.35±5.32)L/min, weaning time (6.84±2.07)d, and total oxygen inhalation time(7.22±3.39)d were better than those of the control group(t=3.524, 6.781, 7.056, P<0.05); PaCO2(48.37±2.31)mmHg, PaO2(75.67±3.25)mmHg and P/F (289.75±16.89) indexes of the experimental group after treatment were better than those of the control group(t=7.161, 8.934, 11.821, P<0.05). Conclusion The non-invasive intermittent positive pressure ventilation (NIPPV) mode in the treatment of neonatal respiratory distress syndrome is significantly better than the nasal continuous positive airway pressure (N-CPAP) mode, which effectively improves the patient’s lung function and blood oxygen status, and reduces the weaning time and the total oxygen inhalation time, it is recommended for clinical application.
