[摘要] 目的:探讨临床护理路径在原发性开角型青光眼患者手术中的应用效果。方法:将30例行复合式小梁切除术的青光眼患者列为观察组,实施临床护理路径。将30例已出院行复合式小梁切除术的青光眼患者列为对照组,实施常规护理。比较两组的护理效果。结果:平均术前等待日、平均住院日、平均住院费用,观察组均显著少于对照组,P<0.05,差异有统计学意义;医疗护理质量比较,观察组亦显著优于对照组,差异有统计学意义,P<0.05。结论:应用临床护理路径对行复合式小梁切除术的原发性开角型青光眼患者进行护理,可有效提高医疗护理质量及患者满意度,减少平均住院日,降低医疗成本。

Clinical nursing path in patients with primary open-angle glaucoma surgery evaluation

[Abstract] Objective: To study the effect of clinical nursing path in patients with primary open-angle glaucoma surgery evaluation. Methods:Compound in 30 routine patients with glaucoma trabeculectomy as the observation group, the implementation of clinical nursing path. The 30 cases had been discharged from a combined trabeculectomy in glaucoma patients as a control group, the implementation of routine care. Comparative effects of two groups of care.. Results:The average wait for days before surgery, with an average length of stay, average hospital stay costs, the observation group were significantly lower than the control group, P<0.05, significant difference; the quality of medical care compared to the observation group had significantly superior to the control group, the difference statistically significance, P<0.05. Conclusion: Application of clinical nursing path to a combined trabeculectomy for primary open-angle glaucoma patients with care, can effectively improve health care quality and patient satisfaction, reduce the average length of stay and lower medical costs.

临床护理路径(clincical nursing pathway,CNP)是指依据每日标准护理计划,为一类特殊患者所设定的住院护理图式。由美国护士Kaven Zander 1985年首次提出护理程序与路径的概念[1]。此后,CNP在美、英、澳大利亚、新加坡、中国台湾等国家和地区得以推广应用,提供兼顾效益-成本的高质量治疗护理[2]。本院为探索CNP在眼科临床护理中应用的可行性和效果,进行了原发性开角型青光眼单病种CNP的临床研究,现报道如下:
1 资料与方法
1.1一般资料
将2009年1～12月于本院眼科诊断为原发性开角型青光眼行复合式小梁切除术的患者30例作为观察组,其中,男12例,女18例,年龄37～69岁,平均53.2岁,排除合并糖尿病、原发性高血压及心脏病的患者,实施临床护理路径;2007年1月～2008年12月收集符合纳入和排除标准的原发性开角型青光眼行复合式小梁切除术的出院患者30例作为对照组,其中,男11例,女19例,年龄35～72岁,平均54.5岁。两组患者术前一般情况良好,手术均采用局部麻醉,且年龄、性别、病种等比较,差异无统计学意义(P>0 ......
