[摘要] 目的:探讨大鼠急性重症胰腺炎(SAP)与肠黏膜屏障的关系以及鱼油干预的影响,为临床营养支持提供依据。方法:用3.5%牛磺胆酸钠逆行胰胆管注射法诱导SAP模型。48只成熟SD大鼠造模成功后分为两组,生理盐水组(A组,n=24),鱼油干预组(F组,n=24),分别于造模后给予尾静脉营养支持,与实验后6、12、24 h分别处死各组的其中8只大鼠,观察各自胰腺、小肠病理变化并测定其血清内毒素浓度。结果:F组门静脉血内毒素水平低于A组(P<0.05);F组肠道损伤评分低于A组(P<0.05)。结论:添加静脉鱼油营养支持能降低SAP时门静脉血中的内毒素水平、促进肠道黏膜的修复、加强肠黏膜屏障的保护作用。

[Abstract] Objective: To investigate the protective effect of fish oil on intestinal mucosa barrier in severe acute pancreatitis (SAP) rats. Methods: 48 SD rats were divided into 2 groups at random (A,F), 24 in each group, group A was given saline water, group F was given fish oil, established model of intestinal injury after feeding rats 6 hours, then group A was given saline water, group F was given fish oil untill be killed. To measured concentration of endotoxin (ET) in portal vein blood supernatant after 6, 12 and 24 hours. Results: Serum ET concentration in group F was significantly lower than group A (P<0.05). The degree of injury of intestinal mucosa was much lower in group F than group A. Conclusion: The fish oil can prevent the intestinal mucosa barrier in SAP.

急性重症胰腺炎(severe acute pancreatitis, SAP)在临床上起病凶险,发病率高,死亡率高,引起全身广泛炎症反应,容易导致肠黏膜屏障功能的损伤。鱼油在维持肠黏膜形态、保护肠黏膜屏障、防止细菌易位等方面具有积极的作用。本研究通过对急性重症胰腺炎(SAP)大鼠给予静脉鱼油营养支持,观察其对肠黏膜的保护作用,为临床应用提供依据。
1资料与方法
1.1动物与试剂
健康成年SD大鼠48只,雌雄各半,体重210~240 g。内毒素检测采用鲎试剂偶氮显色法,显色基质鲎试剂盒购自厦门鲎试剂实验厂有限公司。
1.2 动物模型的建立
按人工胰胆管逆行灌注法复制SAP模型,术前12 h禁食,自由饮水,2%戊巴比妥钠腹腔注射麻醉(40 mg/kg),上腹正中切口入腹,轻柔提出十二指肠和游离胰腺,用微动脉夹夹闭肝门处总胆管使之扩张,通过1 ml注射器向胆胰管内匀速注射3.5%牛磺胆酸钠(2 ml/kg),输注结束后留置针头5 min,保持胰管内压力,移去动脉夹后,拔针、关腹、注意无菌操作。
1.3实验分组
48只健康SD大鼠,质量为210~240 g,随机分为生理盐水组(A组,n=24),鱼油组(F组,n=24)。分别通过尾静脉营养输入0.9%NaCl溶液和鱼油。鱼油(奥地利费森尤斯卡比公司制)。术后饲养期间,禁食自由饮水。
1.4检测指标
造模成功、营养支持后,在6、12及24 h随机每组各取8只大鼠抽取门静脉取血及切取胰腺、小肠组织。并于取材完毕后处死相应大鼠。①血清内毒素水平检测:用无热源注射器穿刺门静脉取2 ml全血 ......
