YKL-40与肿瘤相关巨噬细胞在非小细胞肺癌中的表达及预后分析(3)
在本次研究中笔者发现NSCLC患者YKL-40的水平与TAM的表达具有同步性,YKL-40和TAM之间存在正相关。因此,NSCLC患者体内的YKL-40主要来源可能是TAM。从这个意义来推测,YKL-40有可能是执行TAM生物学作用的下游蛋白之一。笔者发现,在合并淋巴结转移、浸润较深的肺癌患者中YKL-40表达增高,Ⅲ期YKL-40阳性率高于Ⅰ期和Ⅱ期患者;随访中发现,部分 YKL-40高表达的患者,术后早期复发的情况较多见;提示YKL-40增高可能是不良的预后因素。病灶局部TAM的水平术前无法测定,术后对于患者体内TAM表达状况仍然无法直接获得,但是术前和术后YKL-40的水平可以通过ELIAS进行测定。对于YKL-40水平明显增高的患者,笔者可以看作为体内TAM活动的指标,作为一种高危因素应注意加强术后的辅助治疗和动态监测,从而对病情作更好的预测,为肺癌的管理提供有力的信息。但本组患者的数量还偏少,临床资料还需要进一步扩大;还需要后续的研究来深化结论。
