人乳头瘤病毒检测联合液基细胞对宫颈上皮内瘤变的诊断价值(1)

[摘要] 目的：探讨宫颈液基薄层细胞学(TCT)检测法联合高危型人乳头瘤病毒杂交捕获法Ⅱ(HC2)对宫颈上皮内瘤变的诊断价值。方法：回顾性分析2008年1月~2011年1月1 557例门诊患者的TCT和HC2检测结果，其中632例TCT检查异常者行阴道镜下多点活检。结果：1 557例患者中高危型HPV感染率为36.7%，其中17~30岁年龄组患者的HPV感染率为51.6%，明显高于其他年龄组。TCT检查正常组HPV感染率为26.2%；TCT检查阳性组、LSIL组、HSIL组及SCC组的HPV感染率分别为73.5%、79.7%、86.3%及93.7%，均明显高于TCT正常组。TCT检查阳性者与宫颈活检病理符合率为71.5%；高危型HPV-DNA与TCT检查均为阳性组与病理检查符合率为89.0%。结论：高危型HPV感染与宫颈病变严重程度密切相关，HC2检测联合TCT是筛查宫颈癌前病变的可靠手段。

The value of high-risk HPV detection by Hybrid Capture Ⅱand TCT in screening cervical intraepithelial neoplasma

[Abstract] Objective: To evaluate the value of high-risk HPV (hrHPV) detection by Hybrid Capture Ⅱ(HC2) and thin-prep cytology test (TCT) in screening cervical intraepithelial neoplasm (CIN). Methods: A retrospective analysis was done on 1 557 patients screening with HC2 and TCT. Among them, 632 patients received colposcopy-directed biopsy with abnormal TCT results. Results: Among the 1 557 patients, the incidence of hrHPV infection was 36.7% and had a higher tendency at 17-30 age. The incidence of hrHPV infection with abnormal TCT results, low squamous intraepithelial lesion, high squamous intraepithelial lesion, and squamous carcinoma were 73.5%, 79.7%, 86.3% and 93.7% respectively, significantly higher than 26.2% which was the incidence of hrHPV infection with normal TCT results. The coincidence on diagnosis of CIN between HC2 combined TCT and pathologic examination of biopsy was 89.0%, significantly higher than 71.5%, the coincidence on diagnosis of CIN between TCT and pathologic examination of biopsy. Conclusion: hrHPV infection has a close relation with CIN, increasing along with the severity of CIN. HC2 combined TCT is a reliable method for screening CIN.
