深部热疗联合同步放化疗对中晚期宫颈癌的近期疗效观察(1)

[摘要] 目的：观察深部热疗加同步放化疗治疗晚期宫颈癌的临床疗效及不良反应。方法：80例Ⅱb~Ⅳa期宫颈癌患者，随机均分为深部热疗加同步放化疗组(综合组)和同步放化疗组(对照组)，对照组行适形放疗配合同期化疗，顺铂40 mg/m2单药每周化疗，共6次；综合组放化疗方法同对照组，深部热疗每次治疗60 min，每周1~2次，共8~12次。结果：综合组和对照组治疗结束后肿瘤完全缓解率、1年期无瘤生存率比较，差异有统计学意义(P<0.05)。两组不良反应比较，差异无统计学意义(P>0.05)。结论：热放化综合治疗可以提高患者近期疗效，且无严重的并发症，是中晚期宫颈癌治疗中的一种安全有效的模式。

Evaluate of short-term effect of hyperthermia combined with chemoradiation therapy in advanced cervical cancer

[Abstract] Objective: To evaluate the short-term effect and acute toxicity of SR-1000 raidofrequency combined with chemoradiation therapy in advanced cervical cancer. Methods: Eighty women with invasive squamous cell carcinoma or adenocarcinoma of the cervix of stageⅡb-Ⅳa were enrolled. Patients were randomly divided into two groups who received one of two regimens: hyperthermic chemoradiotherapy (HCR) and chemoradiotherapy(CR). Full-dose radiotherapy and 6 courses of chemotherapy was delivered to all patients according to plan. 8-12 courses of hyperthermia treatment were received by group HCR. Results: The complete remission and 1 year survival rate without cancer was significantly higher in groups HCR than in group CR. Toxicity was fully comparable and tolerant between the two groups. Conclusion: All the results above sugested that the combination of full-dose radiotherapy, chemotherapy, and hyperthermia is feasible and effective in patients with advanced cervical carcinoma. The combination therapy is safe and well-tolerated for advanced cervical cancer.

子宫颈癌是全球妇女中发病率仅次于乳腺癌的恶性肿瘤，在中国发病率一直居妇科恶性肿瘤的首位，死亡率居全身恶性肿瘤第7位[1]。晚期的宫颈癌治疗较为困难，尽管近年来同步放化疗治疗模式对疗效有所改善，但总体5年生存率<40%[2]。热疗近年来广泛应用于临床，在肿瘤治疗中取得较好的疗效，本研究通过随机分组前瞻性探讨深部热疗联合同步放化疗治疗Ⅱ~Ⅳa期宫颈癌的疗效和安全性。
