人工流产术后放置吉妮IUD和爱母IUD的临床效果对比分析

[摘要] 目的 对比分析人工流产后放置吉妮IUD和爱母IUD的临床效果。 方法 选择2009年1月～2011年2月在本院行人工流产术后知情选择并即时放置宫内节育器(IUD)的220例妇女，其中，应用吉妮IUD 121例(Ⅰ组)，应用爱母IUD 99例(Ⅱ组)，随访1年，比较两种IUD的临床效果。 结果 Ⅰ组术后IUD脱落率、因症取出率均明显低于Ⅱ组，差异有统计学意义(P < 0.05)，而续用率高于Ⅱ组，差异有统计学意义(P < 0.05)；两组的带器妊娠率比较差异无统计学意义(P > 0.05)。 结论 两种IUD均安全有效，但吉妮IUD临床效果更好、更安全长效、副作用小、脱落率低、继用率高，更适合人工流产后妇女使用。

The comparative analysis of clinical effects of GyneFix IN IUD and MCu IUD after abortion

[Abstract] Objective To compare and analysis the clinical efficacy of placing Jeanne IUD and loving mother IUD after induced abortion. Methods Two hundred and twenty women with induced abortion were placed IUD by the principle of informed consent from January 2009 to February 2011 in our hospital, they were divided into group Ⅰ (121 cases) and group Ⅱ (99 cases), and the group Ⅰwas placed Jeanne IUD while the group Ⅱ was placed loving mother IUD, all of the women were followed up for 1 year, then compared the clinical efficacy of the two groups. Results The IUD expulsion rate and removal rate due to disease in the group Ⅰ were lower than that of the group Ⅱ, there were significant difference between two groups (P < 0.05) , while the renewal rate in group Ⅰ was significant higher than group Ⅱ, there were significant difference between two groups (P < 0.05); there was no significant difference in the pregnancy rate between the two groups (P > 0.05). Conclusion Both of the two IUDs are safe and effective, however, the Jeanne IUD had higher clinical efficacy, safer and longer term, less side effects, lower loss rate, higher continue using rate and more suitable for the women after induced abortion than the loving mother IUD.

在我国，每年做人工流产手术的女性众多。调查显示[1]，行人工流产手术的女性中，真正医学原因导致的流产只占一小部分，大部分是由于平时忽视避孕所致，还有一部分则是缺乏避孕知识或使用方法不正确。目前我国育龄妇女使用最多的避孕措施便是放置宫内节育器(IUD)[2]。人工流产术后即时为患者放置IUD[3]，可减少带环时的疼痛，并节省时间减轻经济负担。笔者对本院2009年1月～2011年2月人工流产术后知情选择并即时放置吉妮IUD和爱母IUD，且无手术禁忌证的220例妇女进行观察比较，现报道如下：
1 资料与方法
1.1 一般资料
选择2009年1月～2011年2月来本院行人工流产术后知情自愿选择放置宫内节育器(IUD)的220例妇女，按患者意愿分为两组，Ⅰ组放置吉妮IUD 121例，Ⅱ组放置爱母IUD 99例，两组患者经妇科检查一切正常，无手术禁忌证，年龄25～40岁，符合IUD放置条件：平时月经较规律，无生殖器炎症及肿瘤；无异位妊娠史；无产科并发症；无中度或重度贫血；无铜过敏史。两组的年龄、身体状况、孕周等方面比较差异无统计学意义(P > 0.05)，具有可比性。
1.2 方法
两组均在人工流产术后即时放置宫内节育器，置器前用腹壁超声对子宫进行多切面扫描。使用探针探查子宫腔，由受过培训的专业医师，在B超监测下严格按照两种IUD的放置步骤完成手术，并确保置器质量。放置IUD后，观测固定线结在宫底肌层的深度，若线结看不清，以子宫浆膜层距吉妮柔适最上缘1.1～1.7 cm为正确位置。术后随访12个月，采取B超监测，记录2种IUD的临床效果。吉妮柔适IUD由天津和杰医疗器械有限公司生产，含铜表面积330 mm2，有尾丝。爱母IUD由鞍山高新技术产业开发区辽宁爱母医疗科技有限公司生产，含铜表面积110 mm2，呈V型，无尾丝。
1.3 统计学方法
采用SPSS 17.0统计分析软件，计量资料以均数±标准差(x±s)表示，计量资料比较采用t检验，计数资料比较采用χ2检验，P < 0.05表示差异有统计学意义。
2 结果
2.1 两种IUD放置后临床效果比较
