心理状态与器官移植预后相关性的研究进展(2)
1.2.4 肾移植 有关肾移植患者的早期研究发现，移植前抑郁诊断[18]和移植后抑郁症状[19]会增加移植失败的风险。更多近期的研究支持这些观点，其中Novak等[21]研究发现一个包含60个条目的关于抑郁症状的量表中症状每多1条，则5年内的移植失败风险增加3%。
2 人格特征与移植预后
有研究使用DS14量表将51个心脏移植患者分为D型性格或非D型性格，发现D型性格的患者死亡率为33%，非D型性格的患者死亡率为8%，排斥反应的发生率分别为40%和27%，第一次发生排斥反应的平均时间分别为14 d和50 d[22]。还有研究显示D型性格或悲观主义的患者的生活质量各维度得分普遍较低[23-24]，不过乐观主义或悲观主义均与患者存活时间无关[24]。Zhang等[25]也对55个肝移植患者进行了艾森克人格问卷等调查，发现患者人格特征与生活质量相关，但与器官功能无关。
3 小结
器官移植中，心理状态对于器官移植患者预后的影响尚无定论。总之，相比于发生在移植前的心理问题，发生在移植后早期的心理问题与移植术后存活率和术后并发症的发病率更为相关，而患者的人格特征亦与其移植相关预后、生活质量等相关。因此，在最新的证据出现之前，认为应在器官移植前对患者进行精神心理方面的评估，同时在移植后1年内对患者的心理状态进行反复评估，以便捕捉到可能影响长期预后的症状学变化，并及时加以疏导和治疗。
