我院1800张中药饮片处方分析(1)
[摘要] 目的 了解本院中药饮片处方的书写质量情况，分析处方中存在的问题，从而规范处方的书写，促进合理用药，保障医疗安全，提高医疗质量。 方法 抽取中药处方(2014年1～6月)1800张，按不合理处方点评指南进行回顾性分析。 结果 本院中药饮片的合格处方是1710张，合格率为95%，不合格处方90张，不合格率为5%。 结论 本院的处方抽查结果是合格的，在往后的处方点评要进一步做好中药饮片的处方分析，确保处方的书写质量和患者的用药安全、有效，以促进临床合理用药。

Analysis of 1800 prescriptions of medicine materical crude slices of Chinese medicine in our hospital

[Abstract] Objective To understand the writing quality of prescription of medicine materical crude slices of Chinese medicine in our hospital， the problems in the prescription were analyzed to regulate prescription writing and promote rational drug use，guarantee medical safety in order to improve the quality of medical care. Methods 1800 prescriptions were extracted (from January to June 2014)，according to the irrational prescription comment guideline， the prescriptions were retrospectively analyzed. Results Qualified prescription of medicine materical crude slices of Chinese medicine in our hospital were 1710，the qualified rate was 95%，the unqualified prescriptions were 90，the unqualified rate was 5%. Conclusion Prescription sampling results of our hospital is qualified.Analysis of prescription of medicine materical crude slices of Chinese medicine should be done well in subsequent comment of prescription to ensure the writing quality of prescription and safe and effective medication of patients which promotes rational drug use.

本院是广东省先进的中医院，是省中医院的协作医院，在粤西地区有很高的声誉，特别是在中药方面更是龙头单位，每月的中药饮片处方都在3500～4500张。为规范中药饮片处方的书写质量，促进合理用药，本研究随机抽取了本院2014年上半年1800张中药饮片处方，对不合格的中药饮片处方按照《处方管理办法》《医院处方点评管理规范(试行)》和2010年11月颁发的《中药饮片处方格式及书写规范》的要求进行回顾性分析。
1 资料与方法
1.1 一般资料
随机抽取本院2014年1～6月每月的中药饮片处方各300张，共1800张。抽取的处方覆盖了本院各科室，具有一定的代表性。
1.2 方法
根据2010年版《中国药典》《中药饮片处方格式及书写规范》《处方管理办法》《医院处方点评管理规范(试行)》的要求，对所抽查的不合格中药饮片处方、不合理用药情况进行统计分析。
2 结果
2.1 1800张中药饮片处方分析结果
合格的中药饮片处方是1710张，合格率为95%，不合格的中药饮片处方是90张，不合格率为5%，其中门诊60张，病房30张(表1)。

2.2 90张不合格处方原因分析结果
不合格的处方中不规范的处方是53张，占58.89%，用药不适宜的处方是37张，占41.11%(表2)。
