多层螺旋CT与磁共振成像诊断隐匿性骨折的临床价值对比研究(1)
[摘要] 目的 比较多层螺旋CT与核磁共振成像(MRI)诊断隐匿性骨折的临床价值。 方法 选取120例隐匿性骨折患者,随机分为CT组与MRI组各60例,CT组行多层螺旋CT检查,MRI组行MRI检查,分析结果。 结果 CT组隐匿性骨折的检出率为86.67%,MRI组检出率为93.33%,CT组对隐匿性骨折的检出率低于MRI组(P<0.05);CT组隐匿性骨折者中50.00%可见骨皮质与骨小梁同时有中断,26.92%单纯骨皮质中断,19.23%单纯骨小梁中断,53.85%单纯横断面图像显示骨折,46.15%联合冠状面、矢状面或者斜面诊断为骨折;MRI组隐匿性骨折中82.14%表现为T1WI可见骨皮质下条线状低信号、不规则线状显像,23.21%患者T1WI未见低信号,所有隐匿性骨折患者均显示T2WI高信号。结论 MRI诊断隐匿性骨折的显影率高。

Comparative study on clinical value of multi-slice spiral CT and magnetic resonance imaging in the diagnosis of occult fracture

[Abstract] Objective To compare the clinical value of multi-slice spiral CT and magnetic resonance imaging(MRI) in the diagnosis of occult fracture. Methods 120 patients with occult fracture were randomly divided into the CT group and MRI group,and there were 60 cases in each group,the CT group was given multi-slice spiral CT examination,the MRI group was given MRI examination,and the results of the two groups were analyzed. Results The detection rate of occult fracture in the CT group was 86.67%,the detection rate in the MRI group was 93.33%,the detection rate of occult fracture in the CT group was lower than that in the MRI group (P<0.05);in the CT group with occult fracture,there were 50.00% patients with interruption of cortical and trabecular bone at the same time,26.92% patients with simple cortical bone disruption,19.23% patients with simple trabecular disruption,simple cross-sectional image displayed fracture in 53.85% patients,46.15% patients diagnosed fracture by combined with coronal and sagittal or slant.In the MRI group with occult fracture,there were 82.14% patients showed the linear low signal and irregular linear imaging under cortical on T1WI,23.21% patients with no low signal on T1WI,all patients with occult fracture showed high signal on T2WI. Conclusion MRI in the diagnosis of occult fracture has high visualization ratio.

隐匿性骨折(occult fracture)是指在传统普通X线摄片时未见到骨折显像,但是实际存在的骨折,又称为微型骨折[1-3],临床症状不典型,容易出现误诊或者漏诊,部分患者因为X线检查未显示骨折因此正常活动,往往引起隐匿性骨折加重,因此提高隐匿性骨折的诊断率,对于减少误诊、漏诊、患者并发症的发生等均十分必要。多层螺旋CT与磁共振成像(MRI)是临床上除普通X线摄片外最常用的诊断是否存在骨折的检查方法,对隐匿性骨折的诊断确诊率较高[4-6],本研究对120例隐匿性骨折患者分别采用多层螺旋CT与MRI进行检查,旨在探讨多层螺旋CT与MRI诊断隐匿性骨折的临床价值。 (陈冰)
