不同剂量地佐辛联合丙泊酚在无痛人工流产麻醉镇痛中的应用效果比较(1)
[摘要] 目的 探讨不同剂量地佐辛联合丙泊酚在无痛人工流产麻醉镇痛中的应用效果。 方法 选取本院2013年6月～2014年9月门诊收治的240例无痛人工流产手术患者作为研究对象。按照入院顺序随机分为A、B、C组，每组80例。3组患者均采用地佐辛联合丙泊酚复合麻醉。A、B、C组地佐辛剂量分别为0.05、0.075、0.10 mg/kg。比较3组患者的临床麻醉效果及丙泊酚用量，统计不良反应发生率及术后宫缩疼痛情况。 结果 B组的麻醉效果优于A、C组，差异有统计学意义(P<0.05)。B组的丙泊酚用量少于A、C组，B组的术后30 min内宫缩疼痛评分低于A、C组，差异有统计学意义(P<0.05)。B组的并发症发生率低于A、C组，差异有统计学意义(P<0.05)。 结论 在无痛人工流产术中患者应用0.075 mg/kg剂量的地佐辛联合丙泊酚麻醉，既可保证术中、术后的麻醉镇痛效果，又可减少术中丙泊酚用量，降低术后不良反应发生率，值得广泛推广应用。

[Abstract] Objective To explore application effect comparison of different dosage of dezocine combined with propofol in anesthesia analgesia of painless artificial abortion. Methods From June 2013 to September 2014，240 patients undergone painless artificial abortion surgery treated in our outpatient service were selected as research objects.According to the sequence of admission into the hospital，they were evenly divided into group A，B，C in random.Combined anesthesia of dezocine and propofol among 3 groups was all applied.In group A，the dose of dezocine was 0.05 mg/kg，and it was 0.075 mg/kg in group B，and 0.10 mg/kg in group C.The clinical anesthetic effect and dosage of propofol among 3 groups was compared respectively，and incidence rate of adverse reaction and pain in uterus contraction was calculated respectively. Results The anesthetic effect in group B was superior to that in group A and group C，and there was a statistical difference (P<0.05).Drug dosage of propofol in group B was less than that in group A and group C.Score of pain in uterus contraction within 30 min after surgery in group B was lower than that in group A and group C，which displayed statistical difference (P<0.05).In group B，incidence rate of adverse reaction was lower than that in group A and group C，and there was a statistical difference (P<0.05). Conclusion In patients with painless abortion surgery，0.075 mg/kg dezocine combined with propofol for anesthesia not only guarantees anesthetic pain relief effect during and after surgery，but also reduce the intraoperative dosage of propofol and postoperative incidence rate of adverse reaction，which is worthy of expansive promotion and application.

临床研究发现[1-2]，丙泊酚作为常用的麻醉药物已被广泛应用于人工流产术麻醉中，其麻醉效果佳，但镇痛效果差，患者术后出现腹痛是常见的术后并发症，为了减少术后疼痛的发生，临床可联合适量镇痛性药物共同用于术前麻醉。本院从2013年6月开始研究不同剂量地佐辛联合丙泊酚对无痛人工流产术患者麻醉镇痛效果的影响，现报道如下。 (张定秀等)
