[摘要] 目的 探讨团队管理模式在糖尿病健康管理中效果。 方法 收集我院收治的300例2型糖尿病患者，随机分为干预组和对照组。对照组采用常规的药物治疗，干预组加用团队模式，观察对比干预前后糖尿病患者的各项指标。 结果 干预组较对照组对糖尿病的认知情况明显升高(P<0.01)；干预组在机体健康各项指标测量改善方面显著优于对照组，差异有统计学意义(P<0.01)。 结论 团队管理模式应用于糖尿病健康管理中能帮助患者较好地认识糖尿病，控制血糖，改变不良行为，为糖尿病的健康管理提供参考依据。
