补佳乐联合维生素E对绝经后子宫脱垂术前阴道壁情况的改善效果(1)
[摘要]目的 探討补佳乐联合维生素E对绝经后子宫脱垂术前阴道壁情况的改善效果。方法 选取60例我院2014年3月～2016年9月期间收治的绝经后子宫脱垂患者作为研究对象，按随机数字表法分为两组各30例。其中30例患者直接行子宫脱垂术(阴式子宫切除术+阴道前后壁修补术)治疗作为对照组，另30例患者术前应用补佳乐联合维生素E治疗，再行子宫脱垂术治疗作为观察组。观察两组患者阴道内条件、治疗情况、并发症情况以及生活质量情况等。结果 治疗前两组患者MI、KI、阴道黏膜厚度、FSH以及E2水平及VHS差异无统计学意义(P>0.05)；治疗后观察组上述指标均优于对照组，差异有统计学意义(P<0.05)。对照组治疗前后仅VHS评分差异有统计学意义(P<0.05)，其余指标差异无统计学意义(P>0.05)；观察组治疗前后所有指标差异均有统计学意义(P<0.05)。观察组子宫内膜厚度与对照组差异无统计学意义(P>0.05)；但手术时间、术中出血量及住院时间均优于对照组，差异有统计学意义(P<0.05)。观察组、对照组并发症发生率分别为3.33%、13.33%，两组对比差异有统计学意义(P<0.05)。术后1周两组患者生活质量差异无统计学意义(P>0.05)；术后1、3个月，观察组生活质量评分明显高于对照组，差异有统计学意义(P<0.05)。结论 绝经后子宫脱垂患者行子宫脱垂术时，术前给予补佳乐联合维生素E治疗可有效改善阴道内环境，调节激素水平，更符合手术条件，减少术后并发症，提高患者术后恢复效果，值得临床应用及推广。

[Abstract]Objective To investigate the effect of estradiol valerate combined with vitamin E on vaginal wall of postmenopausal uterine prolapse surgery.Methods 60 patients with postmenopausal uterine prolapse admitted to our hospital from March 2014 to September 2016 were selected as study object.They were randomly divided into two groups with a total of 30 cases in each group by random number table method.Among them，30 patients underwent direct uterine prolapse surgery (vaginal hysterectomy + vaginal wall repair surgery) for treatment were served as control group，and the other 30 patients underwent preoperative estradiol valerate combined with vitamin E and uterine prolapse surgery for treatment were served as the observation group.Endovaginal condition，treatment condition，complication and quality of life，etc.of patients in two groups were observed.Results The level difference for MI，KI，VHS，vaginal mucosal thickness，FSH and E2 for patients in two groups before treatment were compared with no difference (P>0.05)，the difference was statistically significant between two groups after treatment (P<0.05).There was no statistical difference in control group before and after treatment besides VHS (P>0.05)；there were significant differences in observation group before and after treatment for all indexes (P<0.05).There was no significant difference between the two groups for endometrial thickness (P>0.05).After 1 week of the operation，there was no significant difference in the life quality for patients in two groups (P>0.05).After 1，3 months of the operation，the life quality score of observation group was significantly higher than that of control group and the difference was statistically significant (P<0.05).Conclusion When patients with postmenopausal uterine prolapse undrergo uterine prolapse surgery，the combined offering of progynova with vitamin E before operation can effectively improve the vaginal environment and regulate hormone level.It′s more in line with the operation condition，can reduce postoperative complication and improve the postoperative recovery effect，so that it′s worthy of clinical application and promotion.
