超声新技术在慢性心力衰竭心脏同步性检测中的临床应用价值(1)
[摘要]目的 探讨超声新技术在慢性心力衰竭心脏同步性检测中的临床应用价值。方法 选取2015年1月~2016年10月我院收治的25例慢性心力衰竭并窄QRS波患者作为观察1组,选取同期的25例慢性心力衰竭并宽QRS波患者作为观察2组,并选取同期在我院参加体检的25例健康志愿者作为对照组。比较各组患者的左室基底、中心段十二点长轴速度峰值差值(Ts-6D-12)、间隔后壁达峰时间差值(SPWMD)、左心室十五节段至收缩血量最小值(Tm sv 16-SD%)及手术前后超声指标。结果 观察2组的Ts-SD-12为(131.31±12.17)ms,SPWMD为(154.97±15.97)ms,Tm sv 16-SD为(12.64±4.97)%,均高于对照组,差异有统计学意义(P<0.05),与观察1组比较差异无统计学意义(P>0.05)。且2组患者经治疗的LVEDD、LVEF、Ts-SD-12、SPWMD、Tm sv 16-SD均较治疗前有优势,且差异有统计学意义(P<0.05)。结论 超声新技术在慢性心力衰竭心脏同步性检测中的临床应用价值显著,值得在临床推广实施。

[Abstract]Objective To explore the clinical application value of new ultrasound technology in the cardiac synchronous detection in patients with chronic heart failure (CHF).Methods From January 2015 to October 2016,25 CHF patients with narrow QRS wave were selected into observation group 1.Meanwhile,another group of CHF patients with wide QRS wave were chosen as observation group 2.In addition,25 healthy volunteers for physical examinations during the same period were recruited into control group.The Ts-6D-12,SPWMD,Tm sv 16-SD%,and ultrasonic indicators before and after surgery were compared among three groups.Results In the observation group 2,the Ts-6D-12,SPWMD,Tm sv 16-SD% were (131.31±12.17) ms,(154.97±5.97) ms,(12.64±4.97)%,which were all higher than the control group (P<0.05).The outcomes of these indicators were not statistically different from those in the observation group 1.The LVEDD,LVEF,Ts-SD-12,SPWMD,and Tm SV 16-SD in CHF patients in the two groups were all superior to those before treatment with statistical significances(P<0.05).Conclusion The clinical application value of new ultrasound technology in the cardiac synchronous detection in CHF is remarkable,and is worthy to be popularized in clinical practice.

慢性心力衰竭(CHF)[1],為各种因素引起的器质性心脏病变的最终阶段,发病率近年有逐步升高趋势,随着医疗水平的不断发展,治疗CHF的手段不断完善,其双心室起搏同步化疗、心脏再移植术、左室辅助泵治疗获得了显著的效果,介于心脏同步化治疗(CRT)[2],对于心脏同步运动的恢复具有促进修复作用,如何实施心脏不同步的评估为临床中实施CRT治疗的重难点[3]。本研究为探讨超声新技术在CHF心脏同步性检测中的临床应用价值,选取不同心电动态差异的CHF患者及健康人群作为本次的研究对象,分别行超声新技术诊断,并将其CRT治疗后的各项超声指标进行比较,现报道如下。, 百拇医药(李斌 黄瑛 刘兰)
