优质护理在人工全髋关节置换术中的应用效果(1)
[摘要]目的 探讨优质护理在人工全髋关节置换术中的应用效果，为临床工作提供理论依据。方法 选取我院2014年6月～2017年6月收治的60例人工全髋关节置换术患者作为研究对象，根据住院先后顺序编号，随机分为对照组(30例)和观察组(30例)。对照组实施常规手术护理，观察组实施优质护理配合，观察并比较两组的护理总满意率及手术时间、术中出血量等。结果 观察组的手术时间短于对照组，术中出血量少于对照组，差异有统计学意义(P<0.05)。观察组护理总满意率为93.33%，显著高于对照组(73.33%)，差异有统计学意义(P<0.05)。结论 在人工髋关节置换术中应用优质护理，可提高护理总满意率，并可缩短患者手术时间，减少术中出血量，有助于提高患者依从性，可在临床推广应用。

[Abstract] Objective To explore the value of high-quality nursing in total hip replacement and provide theoretical basis for clinic. Methods A total of 60 patients who accepted total hip replacement in our hospital from June 2014 to June 2017 were selected as study subjects and divided into control group and observation group according to the admission sequence， 30 cases in each group. Patients in the control group were treated with routine operation nursing， while those in the observation group were treated with high-quality nursing cooperation. Total satisfaction rate of nursing，duration of operation and intraoperative blood loss were observed and compared between the two groups. Results The duration of operation of the observation group was shorten than that of the control group， while intraoperative blood loss was less than that in the control group， the differences were statistically significant(P<0.05). The total satisfaction rate of nursing of the observation group was 93.33%， which was significantly higher than that in the control group (73.33%) (P<0.05). Conclusion High-quality nursing in total hip replacement can improve total satisfaction rate of nursing， shorten the duration of operation and reduce intraoperative blood loss， which is benefit to improve the patients′ compliance. It is worthy to be applied generally in clinical practice.

人工全髋关节置换术是利用金属或高分子聚乙烯、陶瓷等材料制成的人工关节代替病变的关节[1-2]，针对治疗髋臼破坏或退变、股骨头坏死、股骨颈骨折、类风湿性髋关节炎、髋关节骨性关节炎等实施的手术方式，已被广泛开展并取得较好的临床效果，但由于手术操作复杂，手术难度较大，术中要医护配合以提高手术质量、优化手术时间[3]。优质护理是指以患者为中心，强化基础护理，制定方便措施，提高服务质量的护理模式。对人工全髋关节置换术的患者实施优质护理，可降低手术并发症、进而提高患者对护理的满意度[1]，具有重要的临床意义。本研究选取在我院住院接受人工全髋关节置换术的60例患者作为研究对象，探讨优质护理配合在该术式过程中的应用价值，现报道如下。
1资料与方法
1.1一般资料
选取2014年6月～2017年6月在我院住院接受人工全髋关节置换术(均为单侧手术)的60例患者作为研究对象，根據住院先后顺序编号方式分为对照组和观察组，每组各30例。对照组中，男18例，女12例;年龄为60～82岁，平均(68.55±5.26)岁;股骨头坏死14例，股骨颈骨折10例，髋关节骨性关节炎3例，其他3例。观察组中，男16例，女14例;年龄为63～83岁，平均(69.41±4.65)岁;股骨头坏死13例，股骨颈骨折13例，髋关节骨性关节炎2例，其他2例。两组的性别、年龄及疾病类型比较，差异无统计学意义(P>0.05)，具有可比性。纳入标准：①所有患者经检查明确为髋关节病变[4];②均具备人工全髋关节置换术的手术指征;③签署知情同意书自愿加入本研究。排除标准：①年龄<60岁或>82岁，②合并有严重心血管疾病、凝血功能障碍、传染性疾病者。本研究经医院医学伦理委员会审核批准。, 百拇医药(王宝映 黄梅花 冯焕芳)
